Job Description/Essential Functions:

Horizon House, Inc. is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues, and substance abuse. Horizon House is Pennsylvania's oldest psychiatric rehabilitation agency. Since 1952, we have been dedicated to helping people with mental illness, drug and alcohol addictions, developmental/intellectual disabilities, and homelessness to stabilize their lives and to help them live as independently as possible in the community. With over 100 locations throughout South and North Eastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strength and choice.

We are seeking qualified candidates for the position of Residential Coordinator. The position has responsibility for coordinating and providing guidance, counseling, and encouragement to clients as they transition form the mental health system to a more independent environment.

Additional responsibilities include: Responsible for the operation of the site during assigned shift and/or in the absence of the Manager. Participate in the resolution of crisis situations; identify key individuals or resources that can provide direct support and/or immediate intervention during a crisis situation. Ensure the overall safety of assigned individuals by maintaining a hazard free environment avoiding potentially dangerous situations. Maintain and monitor medications consistent with individuals' self-medication plans, in compliance with State and Agency medication administration regulations. Serve as an advocate and encourage residents or tenants to assume a proactive role in their rehabilitation process. Provide appropriate intervention and coverage, to maintain the smooth transition from one shift to another and/or the change to day programming. Monitor performance and evaluation of counseling staff, as directed by the Manager. Monitor the supply and storage of household and necessary food supplies. Provide guidance, instruction and orientation to newly hired or assigned staff, as delegated by the Manager to agency's policies, procedures and regulations.

Job Qualifications:

An Associate's degree in the Social Sciences, healthcare or a related field and 2-3 years of relevant experience in the MH/MR field; or any combination of experience, education or training that would provide the level of knowledge, skill and ability required. Good communication skills, both oral and written. Ability to work independently and as part of a team. Some knowledge of mental health systems and supports. Prior supervisory experience, helpful. Responsible for the operation of the site during assigned shift and/or in the absence of the Manager. Provide appropriate intervention and coverage, to maintain the smooth transition from one shift to another and/or the change to day programming and then back again to the site.

Scope:

Requires the ability to lift, carry, fold, kneel, reach, stack, stoop and bend which may include direct intervention with residents. Possess manual dexterity and fine motor skills. May be required to possess a valid driver's license and an acceptable driving record (contingent upon facility location and client and program needs). May require crisis intervention in emergency situations. May be required to report to work during emergencies, including inclement weather.
