Join our team as a Registered Nurse (RN) and provide top-tier home care services. Leverage your nursing skills to assess patient needs, develop care plans, and administer treatments. Collaborate with physicians and community health professionals to deliver comprehensive care.
Responsibilities:
- Completes an initial assessment of patient and family to determine home care needs.

``` - Initiates the plan of care and makes necessary revisions as patient status and needs change

``` - Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.

``` - Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician.

``` - Participates in on-call duties as defined by the on-call policy.

``` - Instructs, supervises and evaluates home health aide care provided every two (2) weeks.

Qualifications:
- Graduate of an accredited nursing school

``` - Licensed RN in the state of PA
```{=html}
``` - Valid driver's license and reliable transportation
```{=html}
``` - 1-2 years of acute care experience in an institutional setting

``` - Current state licensure and CPR certification
```{=html}
``` - Bachelor's degree and home health care experience preferred
