Job Description/Essential Functions:

Horizon House, Inc. is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues, and substance abuse. Horizon House is Pennsylvania's oldest psychiatric rehabilitation agency. Since 1952, we have been dedicated to helping people with mental illness, drug and alcohol addictions, developmental/intellectual disabilities, and homelessness to stabilize their lives and to help them live as independently as possible in the community. With over 100 locations throughout South and North Eastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strength and choice. We are seeking qualified candidates for the position of Lead Psychiatric Nurse. The position provides support and back up to the Assertive Community Treatment (ACT) Director. As the Lead Nurse. The position has principal responsibility for conducting medication, pharmacy and other medical services activities; continuously monitoring medication practices and preparing the quality assurance plan for medication and medical services and for managing the medication and medical on-duty schedule. Coordinate the behavioral and physical health care needs of service. Monitor participant's medication and provide psychiatric nursing services consistent with physician orders and standard nursing practices. Participate as a member of a focused team which provides Assertive Community Treatment services to ensure that service participants reach their maximum level of functioning in the community. Approximately 75% of the work is in the community. Assist with the development of the ACT program budget including line items to purchase furniture, equipment, communications devices (e.g., telephones, cellular phones, beepers) and supplies and to cover travel and transportation (e.g., purchase or lease program cars, or cover mileage and insurance for use of personal staff vehicles or both). Maintain ACT headquarters with suitable furniture, office equipment, and telephone lines. Provide on-call crisis intervention covering nighttime hours and serve as a backup to evening and weekend staff. Document client progress to maintain a permanent record of client activity and medical concerns according to established methods and procedures. Under the direction of the team psychiatrist and in collaboration with other registered nurses on the team. develop, revise, maintain, and supervise team psycho-pharmacologic and medical treatment and medication policies and procedures including transcribing, administering, evaluating, and recording psychotropic medication prescribed by the team psychiatrist, evaluate and chart psychotropic medication effectiveness, complications and side effects; and arrange for required lab work according to protocol.

Job Qualifications:

Masters, Bachelors or Associates Degree in Nursing, Board Certified Licensed RN in the State of PA with at least one year of nursing experience in an inpatient or outpatient psychiatric setting. Strong clinical skills and experience providing treatment and establishing supportive trusting relationships with persons with severe and persistent mental illnesses while respecting the client rights and personal preferences in treatment. Comprehensive knowledge of psychiatric clinical and psychosocial rehabilitation practices. Excellent communication skills, both oral and written. Strong interpersonal, organizational and analytical skills. Ability to work independently and as part of a focused team. Possession of a valid driver's license with an acceptable driving record Must be available for scheduled emergency on-call service.

Scope: Frequent talking and listening. Requires the ability to lift, carry, fold, kneel, stack, stoop and bend which may include direct intervention with clients. May require verbal crisis intervention assistance in emergency situations. Possess manual dexterity and fine motor skills. Local travel required, including transporting program participants Flexibility in scheduling and available to do 24 hour on-call coverage. May be required to report to work during emergencies including inclement weather.
