Join our team as a Medical Social Worker (MSW) and provide top-tier home care services. Leverage your social work skills to complete social evaluations and provides information and referral services for patients and families/caregivers regarding practical and environmental needs. Collaborate with physicians and community health professionals to deliver comprehensive care.
Responsibilities:
- Assesses the psychosocial status of patients related to the patient's illness and environment and communicates findings to the registered nurse.

``` - Provides information to patients or families/caregivers and community agencies.

``` - Assists physician and other team members in understanding significant social and emotional factors related to health problems

``` - Maintains collaborative relationships with organization personnel to support patient care.

``` - Maintains and develops contracts with public and private agencies as resources for patient and organization personnel.

Qualifications:
- Graduate of a master's program in social work accredited by the Council on Social Work Education.

``` - Minimum of one (1) years' experience in health care.

``` - Experience in a home health care preferred.

``` - Licensed driver with a reliable automobile

``` - MSW License in the State of PA- REQUIRED
