Job Description/Essential Functions:

Horizon House, Inc. is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues, and substance abuse. Horizon House is Pennsylvania's oldest psychiatric rehabilitation agency. Since 1952, we have been dedicated to helping people with mental illness, drug and alcohol addictions, developmental/intellectual disabilities, and homelessness to stabilize their lives and to help them live as independently as possible in the community. With over 100 locations throughout South and North Eastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strength and choice.

We are seeking qualified candidates for the position of Mental Health Specialist. The position has responsibility for providing clinical supervision in assessment and treatment planning, and provides support and back-up to the Team Leader. Provide service coordination (case management for an assigned group of participants) including the coordination and monitoring of the activities of the individual treatment team. Services include home-based visitations and other services within the Philadelphia community. Approximately 75% of work is performed in the community. Provide individual supportive therapy (e.g., problem solving, role-playing, modeling and support), social skill development, and assertiveness training to increase participants' social and interpersonal activities in community settings. Provide services in compliance with contract and Agency regulations. Other responsibilities include the following:

*Provide service coordination (case management for an assigned group of participants) including coordinating and monitoring the activities of the individual treatment team; assume responsibility for developing, writing, implementing, evaluating and revising overall treatment goals and plans in collaboration with the participant and the ITT; provide individual supportive therapy, psychotherapy, and symptom management, ensuring immediate changes are made in the treatment plans as participants' needs change. *Educate and support participants' families, and advocate for participants' rights and preferences. *Perform shift management in coordination with other ACT shift managers according to established policies and procedures. *Submit accurate and timely documentation and related information in accordance to TCMU/CBH/HH standards. Document all contacts, movement, growth, transfers and overall progression through the system, in accordance with funding source regulations. *Consistently meet daily productivity standards as required by the funder and department. *Conduct comprehensive assessment of psychiatric history (e.g. onset, course and effect of illness, past treatment and responses, and risk behaviors), mental status, and diagnosis; physical health and dental health; use of drugs or alcohol; education and employment, social development and functioning; activities of daily living (e.g., self-care, living situation, nutrition, money management); and family structure and relationships. *Provide individual supportive therapy (e.g., problem solving, role-playing, modeling and support), social skill development, and assertiveness training to increase participants' social and interpersonal activities in community settings. *Assist clients in the housing process, including procurement of necessary documentation, completion of housing applications, preparation for move-in, and support after move-in. *Provide ongoing assessment of participants' mental symptoms and partic pants' response to treatment. Make appropriate changes in treatment plans to ensure immediate and appropriate interventions are provided in response to changes in mental status or behavior that put participants at risk (e.g., suicidality).

Job Qualifications:

*Ideal candidate will have a Master's degree in Social Sciences, Healthcare or related field and Post-secondary school education and training is required, including internships and other supervised practical experiences in a clinical or rehabilitation setting with persons with severe and persistent mental illness. *Must be able to communicate both verbally and in written form with every level of the behavioral health system and maintain effective professional relationships with associates in the Agency/community at large. *Skills and competence to establish supportive trusting relationships with persons with severe and persistent mental illnesses and respect for participant rights and personal preference in treatment are essential. *Ability to work independently and as part of focused team. *Working knowledge of Outlook, MS Word and Excel. *Ability to maintain daily productivity standards. *Fee-for-service and electronic health record experience strongly preferred. *Valid driver's license with an acceptable driving record required.

Scope: *Frequent talking and listening. *Requires the ability to lift 25 pounds, carry, fold, kneel, reach, stack, stoop and bend which may include direct intervention with participants. *Possess manual dexterity and fine motor skills. *Must be available for local travel and possess a valid driver's license with an acceptable driving record. *Must carry a mobile phone during scheduled working hours and scheduled on-call services. *As essential personnel, may be required to report to work during emergencies including inclement weather. *Requires crisis intervention and availability to respond in emergency situations.
