Confidential Employer Home Care Manager in Philadelphia, Pennsylvania

POSITION SUMMARY

The Home Care Manager oversees and manages the delivery of home care services to clients. This role involves coordinating care plans, supervising staff, ensuring compliance with health and saety regulations, and maintaining high standards of client care.

KEY RESPONSIBILITIES

1) Client Care Management:

  • Assess clients' needs and create individualized care plans.
  • Monitor and evaluate the effectiveness of care plans, making adjustments as necessary.
  • Ensure clients' care needs are met in a timely and professional manner.
  • Serve as the primary point of contact for clients and their families.
  • Providing quality care to patients
  • Creating incident reports

2) Staff Supervision and Coordination:

  • Recruit, train, and supervise home care staff; including home health aides, and other caregivers.
  • Assign staff to clients based on their needs and caregivers' qualifications.
  • Ensure office compliance.
  • Conduct regular performance evaluations and provide ongoing training and support to staff.
  • Manage scheduling to ensure adequte staffing levels.
  • Maintain office workflow.
  • Working with CHC Insurances.
  • Ensuring Insurance Authorization.

3) Compliance and Quality Assurance:

  • Ensure compliance with all relevant health and saftey regulations including HIPAA and DHS.
  • Maintain accurate and up-to-date records for clients and staff.
  • Implement and monitor quality control measures to ensure the highest standards of care.
  • Conduct regular audits and inspections of care practices and documentation.

4) Budget and Resources Management:

  • Develop and manage budgets for home care services.
  • Monitor expenses and ensure cost-effective delivery of care services.
  • Oversee the procurement and maintenance of ncessary supplies and equipment.
  • Assisting with payroll.

5) Communication and Coordination:

  • Maintain regular communication with clients, families, and healthcare providers.
  • Coordinate with external healthcare providers and services to ensure comprehensive care.
  • Resolve any issues or concerns related to client care promptly and professionally.
  • Assist with the in-take process of on-boarding clients.

6) Reporting and Documentation:

  • Prepare and submit regular reports on the status of clients, staffing, and care delivery.
  • Maintain accurate documentation of all interactions, care plans, and changes in care.
  • Creating incident reports

7) Professional Development:

  • Stay updated with the latest best practices in home care and healthcare management.
  • Attend relevant training sessions, workshops, and conferences to enhance skills and knowledge.

QUALIFICATIONS

Education: HS Diploma; and post HS credits in Healthcare Administration, or related field is a plus

Experience: Minimum of 2-4 years of experience in home care, healthcare management, or a related field. Experience in a supervisory or management role is preferred.

Skills:

  • Proficient in Electric Visit Verification (EVV).
  • Strong leadership and organizational skills.
  • Excellent communication and interpoersonal abilities.
  • Skilled in HHA exchange software.
  • In-depth knowledge of healthcare regulations and best practices.

Work Enviornment:

  • Office-based with regular visits to clients' homes.
  • May require on-call availability for emergencies or urgent care needs.
  • Collaborative work environment with interaction with various healthcare professionals.
