Family Practice & Counseling Network Direct Support Professional - Harmony in Philadelphia, Pennsylvania
Reference #: 2154816 Description SIGN ON BONUS: $500.00 (gross) for full-time staff and $250.00 (gross) for part-time staff. Receive the first 1/2 amount following New Hire Orientation/Onboarding, then the second 1/2 following successful completion of 90-day New Hire Probation.

The Direct Support Professional is responsible for the safety and supervision of our consumers with Intellectual Developmental Disabilities and behavioral health histories to facilitate healthy and socially acceptable behaviors while living in community-based sites. You will coordinate with clinical staff, implement residentially-based interventions, and assist the clients in achieving their clinical assignments. You are responsible for fulfilling the obligations as outlined in the client's program plan. You will prompt or assist, as necessary, to ensure the clients participate in the activities of daily living, in both the home and the community. Assisting in Community Integration Activities and access to community Supports including medical appointments, social security access, etc. Completing all other miscellaneous tasks as assigned by the supervisor Requirements High School diploma or GED is required, associates or bachelor's degree is preferred Basic computer and internet literacy with the ability to utilize applications like emails, time recording and other programs to enter and retrieve data as necessary to perform the essential functions of the job. Prior experience in the human services field, preferred. Valid driver's license Subject to employment screenings, including FBI clearance. RHD employees and programs in the City of Philadelphia are required to follow the COVID vaccine mandate. This mandate became Effective on August 16, 2021 and requires Healthcare Workers and certain workers at Healthcare Institutions to be fully vaccinated by October 15, 2021. Fully vaccinated means it has been two weeks since the final dose of vaccine. The regulation does allow individuals to apply to RHD for an exemption based on a sincerely held religious belief that would prohibit vaccination for COVID-19 or because of medical conditions that preclude administratio of one of the available vaccines.
