Job Description/Essential Functions:

Horizon House, Inc. is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues, and substance abuse. Horizon House is Pennsylvania's oldest psychiatric rehabilitation agency. Since 1952, we have been dedicated to helping people with mental illness, drug and alcohol addictions, developmental/intellectual disabilities, and homelessness to stabilize their lives and to help them live as independently as possible in the community. With over 100 locations throughout South and North Eastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strength and choice.

We are seeking qualified candidates for the position of Case Manager to provide case management and engagement and services to homeless individuals with behavioral health needs as identified by the County Office of Mental Health; and to assess and identify a client's needs in order to provide assistance and referrals to housing and treatment on an on-going basis; to ensure that individuals receive all the benefits for which they are entitled, such as financial, medical, vocational and educational.

Additional responsibilities include:

To implement the overall mission, values and beliefs of Horizon House in all aspects of the job. Provide intake and assessments for clients to determine service needs and eligibility. Inform clients of available and acceptable service intervention options. Coordinate referrals with other agencies/facilities to meet the assessed needs of the clients. Perform Case Management to homeless individuals as identified by OBH by utilizing local resource networks for referrals. May provide outreach and engagement services by working split and/or evening shifts. Work closely with all cooperating agencies, housing and treatment facilities and advocacy or support groups with whom placements have been secured; emphasize commitment and assistance to clients upon discharge or in the event of a relapse into homelessness or substance abuse throughout the recovery process. Maintain and submit all required documentation, on a timely basis, including individual client files intake assessments, movement through the service delivery system and the number of contacts made during outreach.

Job Qualifications:

A.A. degree Bachelor's preferred in Human Services/Social Sciences, Healthcare or a related field and 2 years of experience in substance abuse counseling; or any combination of experience, education or training that would provide the level of knowledge, skill and ability required. Certified Addiction Counselor (CAC), preferred or on-going coursework in Drug and Alcohol Counseling to qualify for certification. Mental Health knowledge desired Knowledge of multiple support systems for substance abusers and familiarity with recovery models. Very good communication skills, both oral and written. Demonstrated ability to be flexible and creative in meeting the needs of clients while maintaining professional boundaries.

Scope: Frequent talking and listening. Requires the ability to lift, carry, fold, kneel, reach, stack, stoop and bend which may include direct intervention with clients. May require crisis intervention in emergency situations. Possession of a valid driver's license and the ability to drive a passenger van. May require working varied shifts to fulfill the mandate of the Hope Caf .
