Finger Lakes Health SOCIAL WORKER MSW in Penn Yan, New York
This position works at both Geneva General Hospital and Soldiers & Sailors Memorial Hospital.
Education:
Minimum : Master's degree in Social Work (Bachelor's degree in Social Work with related work experience will be considered).
License/Certifications: LMSW or eligible is required. Must become licensed. Licensed Master Social Work (LMSW) is preferred.
Experience: Minimum : New graduates considered. Preferred : Previous related experience with Discharge Planning and/or Care Management. Experience in Acute Hospital setting and/or Nursing Home knowledge.
Other: Minimum : Demonstrated ability to handle confidential information with discretion and ability to deal with the people in a professional and courteous manner. Ability to meet deadlines, manage multiple priorities and enhance the spirit of teamwork through effective role modeling. Excellent interpersonal, communication and organization skills. Computer literacy. Preferred : Experience with Microsoft Office products and electronic medical record.
