Title : Peer Outreach Coach (2022-046-1569)

Location: Yates Recovery Center, Penn Yan, NY

Schedule: Monday-Friday 8:00am-4:00pm or 9:00am-5:00pm

Salary: Salary pay range is min. $16.38 to a max. $21.33 per hr. based on education & experience

    Essential Job Functions Overview

    The individual will use their personal story and experiences, with behavioral health services, to help guide members on their path to recovery. Perform a wide range of recovery-based services, assist with vocational/educational training and skills training. Utilize a company owned car or van (14 passenger), and report functions related to the day-to-day operations of the Recovery Center/Drop-in programs. Peer will strive to create a healing environment that respects the perspectives and experiences of the individuals, families, staff, and communities we serve by practicing safe, respectful communication as well as respecting individuals’ boundaries and differences.

  • Maintain attendance records, document interactions, and input progress notes.

  • Transport consumers to and from activities based on a predetermined schedule.

  • Provide outreach services to help individuals link to meaningful activities within their community and the Recovery Center and/or Drop-In programs.

  • Assist residents in developing and maintaining a structured daily schedule according to individualized goals and preferences using trauma informed practices.

  • Utilize motivational interviewing (MI) techniques to provide individual support, training, and assistance in working towards achieving one’s personal goals.

  • Encourage and participate in social, recreational, educational, and cultural activities.

  • Provide resident transportation via Agency vehicle as needed.

  • Assist in general program housekeeping to include provision of hands-on assistance to teach the skills needed to maintain personal living spaces.

  • Develop person-centered service plans for the individuals on your caseload.

  • Assist residents in monitoring progress on stated goals and provide updates regularly.

  • Document interactions with residents as it relates to their service plan goals.

    Experience, Education, & Physical Qualifications

    High School Diploma or equivalent and a minimum of 1 - 3 years’ experience working with special needs consumers in a recovery-based or vocational/rehabilitative setting. Identify as an individual living and/ or receiving behavioral health recovery experiences required. Completion of Peer Certification Program is required within one (1) year from date of hire. Possession of the CPRP credential is a plus. Must have a valid NYS Driver License. Experience with Microsoft Office 365.
