Brookhaven Memorial Hospital Medical Center Licensed Practical Nurse - Homecare - Part Time - Day Shift in PATCHOGUE, New York

  • Provides skilled services to LI Community Hospital Home Care or Hospice patients, under the direction of the RN, as evidenced by:

    • Adherence to infection control practices and agency policies and procedures
  • Use of chronic disease management and health promotion strategies

  • Use of motivational interviewing techniques

  • Use of telehealth

  • Demonstration of observation, measurement, recording and reporting indications of the patient’s health status, medication administration and performance of direct patient care

  • Maintains an average productivity of 6 visits or greater per 7.5 hour day and functions as a weekend and holiday staff back up

    • Participates in care management and demonstrates effective and timely communication as evidenced by;
  • Attendance at staff. team and other mandatory meetings

  • Interacts with interdisciplinary team regarding those patients visited

  • Effective use of voice mail and e-mail

    • Demonstrates timely documentation as evidenced by:
  • Use of the computer in the home

  • Notes are locked and synchronized for processing within 24 hours of visit

  • Timely submission of requests for insurance authorization when directed by the RN case manager

  • Clear documentation of client’s progress towards goals and response to interventions

  • Demonstrates commitment to their team and LONG ISLAND COMMUNITY HOSPITAL Home Health Agency and/or Hospice as evidenced by:

Supports and promotes the mission of LONG ISLAND COMMUNITY HOSPITAL Home Health Agency and/or Hospice

Represents the agency in a professional manner and follows the uniform policy

Flexibility in scheduling with a focus on meeting patient needs

Participation in continuing education (minimum of 3 hours per calendar year), attending annual competency and all mandatory in-services

Attendance at team meetings

Employee adheres to all applicable federal, state, local laws and regulations.

  • Performance Improvement

Participates in research programs and studies. Implements performance activities and participates in related committees under direction of Registered Nurse.

Maintains knowledge of current trends and developments in the field by reading appropriate books, journals and other literature and attending related conferences, seminars and the like.

Accepts assignments based on demonstrated competency to provide care to age and gender specific population.

Participates on departmental committees

Exhibits a working knowledge of the department’s performance improvement functions by completing required logs, understanding and gathering data for studies and the like.

Understands Performance Improvement indicators and actively participates in achieving positive outcomes.

Complies with Department of Health regulations regarding care to patients’ and documentation of it as to not put the agency into a deficiency mode.

Professional knowledge of nursing theory, practice and administration. New York State License and current registration as a Licensed Practical Nurse Current CPR/BLS One to two years’ prior related experience. Advanced analytical skills. Interpersonal skills. Ability to concentrate and pay close attention to detail. Ability to walk and stand continuously (up to 80% of work time) and lift and position patients. Current New York State driver’s license Ability to navigate/travel in Suffolk County. Able to comply with the New York State regulatory requirements to provide patient care Able to comply with all Long Island Community Hospital Home Care and Hospice policies, procedures and standards of conduct Previous experience as an LPN Ability to function as an LPN, under the direction of a Registered Nurse and to work well with others.
