Job Summary:

Provides assessment, treatment, planning and education to persons with disabilities or with chronic/acute illnesses.

Job Functions:

Conducts assessment, treatment, planning and education to address the needs of patients and caregivers.

Based on results of assessments, plans and carries out individually designed treatment programs to improve or restore phsycial function or alleviate pain.

Documents all assessments and treatments according to policy, including maintaining required prescriptions and signatures.

Evaluates effectiveness of treatment and adjusts accordingly in order to achieve maximum benefit.

Instructs patient and caregivers in home exercise programs and provides education as needed.

Discharges patients from therapy when goals are met or projected outcomes have been achieved.

Teaches and supervises therapy students.

Directs the practice of support staff in order to achieve established patient treatment goals.

Education:

Bachelor' s Degree in Physcial Therapy

Current KY Physical Therapy license

BLS certification through the American Heart Association
