Homelessness is the largest social and public health crisis in California. Illumination Foundation (IF) is a growing non-profit organization dedicated towards disrupting the cycle of homelessness by providing targeted, interdisciplinary services in our recuperative care centers, emergency shelters, housing services and children's and family programs. IF currently has 13+ facilities with 22+ micro-communities scattered across Orange County, Los Angeles County and the Inland Empire.
Job Description
The ECM Supervisor in Orange County will be responsible for overseeing day-to-day operations of the Enhanced Care Management program. This includes managing all program roles such as Program Coordinator, Lead Care Managers and Community Health Workers (outreach staff).
The functions of the role include quality of personnel work, from programmatic/contractual perspectives to client-centered and ethics, their programmatic intersection with the Clinical Consultant, ensuring staff productivity, administering corrective action when program standards are not met, and assisting the ECM Director in administrative tasks/functions.
Responsibilities
Supervision
Evaluate the effectiveness of care management services by tracking outcomes, gathering feedback, and implementing necessary improvements.
Monitor performance by means of productivity tracking of Outreach Lead Care Manager (LCM), Community Health Workers (CHW), LCM, Program Coordinators (PC), and Medical Coordinators.
Track and enhance program enrollments.
Oversee the daily activities of the care management team, providing guidance and support to ensure effective care management.
Evaluate the effectiveness of care management services by tracking outcomes, gathering feedback, and implementing necessary improvements.
Provide training and professional development opportunities for care managers to enhance their skills in care coordination and client engagement.
Conduct routine audits of work to maintain billing and compliance purposes.
Attend Health Plan meetings.
Assessing and reporting program outcomes such as enrollments, disenrollments, and engagement.
Build on existing workflows, policies, and general program developments.
Documentation
Work with the ECM Director to ensure that the quality and quantity of ECM staff documentation is up to internal and contractor standards
Identify accurate data / reporting figures, to ensure compliance with contractor program parameters.
Administrative Oversight:
Attend all monthly meetings with contractors and relayed to staff accordingly
Oversee employee timesheets and will be responsible for their compliance / accuracy
Responsible for administering corrective action when expectations / standards are not met by staff members
Responsible for monitoring and updating Clinical and Engagement Reports from Health Plan

Expectations
Knowledge and applied practice of HIPAA parameters
Ability to complete 5% of Members assigned for outreach
Post documentation within 24 hours of encounter
Competency in utilizing databases such as HMIS, CHAMP, LANES, and Collective Medical
Professional honesty, transparency, and fluidity in all client engagements
Professional and discreet interpersonal engagement with all established and prospective enrolled Members as well as fellow care team members
Adherence to Illumination Foundation’s policies and guidelines
Travel to community events and engagement activities such as presenting to prospective community partners or internal facilities
Required:
BS/BA diploma in management, social services, psychology, or a related discipline
Proven experience in fields such as Care Management, Behavioral Health, supervisory or leadership role, work with complex cases, or other related expertise
Strong leadership and team management skills to motivate and develop staff
Excellent communication and interpersonal skills for effective collaboration with clients, families, and stakeholders
Ability to facilitate regular interdisciplinary team meetings to promote information sharing and collaborative problem-solving
Preferred:
Proficiency in assessment and planning tools, as well as case management software
Awareness of and sensitivity to the diverse cultural and socio-economic backgrounds of clients and families
Credentials or training in trauma-informed care, cultural competence , evidence- based practices for care management, and/ or healthcare best practices.
