Olathe Medical Center Clinical Documentation Specialist - Ambulatory & Outpatient in Olathe, Kansas
Job Summary: The Clinical Documentation Specialist is responsible for reviewing patient medical records, EMR, in the Outpatient and Ambulatory setting to ensure the capture of accurate documentation to facilitate proper coding and reimbursement. The position has clinical knowledge and understanding of national coding guidelines and standard of compliance to improve overall quality and completeness of clinical decimation within the patient record. The position works collaboratively with the ambulatory physicians and advanced practice providers to ensure clinical information within the medical record is accurate, complete, and compliant. This includes accurate documentation needed to support the capture of condition categories, ICD-10-CM and medical necessity of ambulatory visits. The Clinical Documentation Specialist will be responsible for educating the patient care teams regarding documentation guidelines, coding requirements and specific service requirements. Minimum Requirements: • LPN, RN, RHIT, CCS, 3 Years Clinical Documentation Improvement • 3 - 5 Years Hands on experience in Medical Record Review • 3 - 5 Years Extensive understanding of coding guidelines, condition categories, age specific categories and documentation needs to support medical necessity, coding, and revenue • 3 - 5 Years’ Experience collaborating and proactively soliciting clarification from physicians, patient care team, to ensure key aspects of care have been properly recorded in the patient’s medical record. Key Responsibilities: • Consistently meets work schedule. • Daily monitoring. • Collaborates with members of the CDI, Case Management and Patient Care Teams in their given environments. • Work collaboratively with clinical leaders, Case Management, PA, CDI and Patient Accounting to support denial prevention • Establishes meaningful communication loops to ensure patient care team members understand issues and resolution opportunities • Provides education to members of the patient care team regarding proper documentation and requirements. • All other duties as assigned.
