Kaiser Permanente Community Health Consultant IV in Oakland, California

Job Summary: Position Intro

Many systems-level efforts are vital to creating good conditions for health, including key city policies that promote population health, research and education on gun violence, strong public health infrastructure, and others. These are amongst some of the key systems-level areas that Kaiser Permanente is focused on for communities. Over the last few years, KP has established critical partnerships and initiatives across the country to promote greater health for our communities that support, strengthen and catalyze city policies for health, further research and education in gun violence, public health infrastructure, and more. We seek a person with experience and skill in managing large scale programs, coordinating and managing projects, with background in public health and/or large scale health-related arenas.

Role

The Community Health Consultant IV will support KP leaders to advance strategies in key public health and systems-level change issues. They will help lead execution across various initiatives, ensuring strong project coordination and implementation. A set of generalist skills and willingness to work at different levels of a project will be essential. A working knowledge in one or a number of different areas/sectors will be greatly beneficial, which could include public health, health/hospital systems, government, academia, community organizations, philanthropy, and others. Our major strategies also involve broad communications and campaigns, with heavy emphasis on developing thought leadership strategies to inform public information and knowledge.

Essential Responsibilities:

  • Promotes learning in others by proactively providing and/or developing information, resources, advice, and expertise with coworkers and members; builds relationships with cross-functional/external stakeholders and customers.   • Completes work assignments autonomously and supports business-specific projects by applying expertise in subject area and business knowledge to generate creative solutions; encourages team members to adapt to and follow all procedures and policies. Collaborates cross-functionally and/or externally to achieve effective business decisions; provides recommendations and solves complex problems; escalates high-priority issues or risks, as appropriate; monitors progress and results. Supports the development of work plans to meet business priorities and deadlines; identifies resources to accomplish priorities and deadlines.   • Implements community health grantmaking and investments by: conducting market charitable contributions committee meetings, training, and orienting committee members; reviewing and synthesizing existing data, evidence, and lessons learned from previous and current grantmaking, the field, policy environment, and other funding initiatives to inform upcoming strategy design; serving as a resource regarding charitable contributions programs/initiatives, including investments, in-kind, and business operational assets, and providing consultation, technical assistance, and assistance in leading basic training sessions as needed; coordinating the development of program evaluations of current grantmaking to ensure and assess of the impact of KPs charitable contribution strategy and working with expert evaluation consultants to develop program evaluations; exercising judgment and discretion in developing grants, including scoping, inviting, analyzing, developing recommendations, presenting to committees; and recommending charitable contributions in alignment with other community health priorities.

  • Creates communication strategies by: initiating and maintaining communications between KP and management in community agencies regarding health programs; communicating KPs position and commitment to community health to internal and external audiences; identifying corporate marketing, community relations outreach, and contributing to media opportunities to increase visibility of community health portfolio and programs; ensuring regular communication with internal and external stakeholders through regular reports and presentations as needed to communicate the impact of community and social health initiatives; developing communications to update programs, related stories, factsheets, press releases, and news articles; representing KP by participating in collaboratives, working groups, advisory boards, committees, panels, conferences, and planning groups; and educating and influencing community leaders by fostering KPs value and commitment to communities served.

  • Conducts community health program management by: managing community health programs in KPs Community Health Program, including planning of initiatives that address community wellbeing; providing insight to ensure that all community health programs and activities meet the needs of KPs diverse communities; leading implementation of community health strategies and programs across the market, including philanthropy, in-kind, and/or community health improvement programs at the local and/or market level; managing, planning, and organizing the market-wide delivery of short and long range community initiatives and focus areas, ensuring alignment with community health priorities and organizational goals; and developing programs that measurably improve community health and support the organizations tax-exempt status.

  • Ensures regulatory compliance by: ensuring that programs and activities are in compliance with legislative, regulatory and KP policies; and coordinating collecting of information to support all regulatory, governmental, and KP reporting requirements.

  • Leverages organizational resources by: utilizing advanced knowledge in leveraging in-kind resources from KP, including technical expertise, programs managed by KP, and operational assets (e.g., hiring, investments, purchasing) to connect to, support, and improve the health of KP communities and organizations and seeking opportunities to advocate bi-directional learning from stakeholders.

  • Builds stakeholder relationships by: building and maintaining relationships with community leaders including non-profits, for profits, elected officials, public agencies, and other key community and internal stakeholders to engage in strategic community health work; engaging market and/or program office leadership and other key internal stakeholders to support execution of community health strategies; collaborating with community health partners to maintain and advance relationships with community organizations to advance KPs mission; organizing work groups involving members of different departments to build and foster partnerships between health plan, care delivery, and community organizations that serve identified populations; and building partnerships with philanthropic, policy, public health, and academic organizations to advance shared missions.

  • Implements strategic planning by: providing input into the development and implementation of community health strategic plans; managing, planning, and implementing national and market community health programs, projects, and initiatives; supporting the strategic development and implementation of a market wide integrated community health plan which supports the organizations strategic imperatives and ensures alignment to local medical service area needs; and supporting market teams to develop and implement innovative approaches to population health improvement.

    Minimum Qualifications:

  • Bachelors degree from an accredited college or university AND minimum five (5) years of experience in government, public health, health services, economic development, community/social/human services, or a related field OR Minimum eight (8) years of experience in government, public health, health services, economic development, community/social/human services, or a related field.
