Support individuals within the ABI/MFP program who live in their own home, assisting with personal care, activities of daily living, individual support, companionship, homemaker, and community habilitation. DUTIES INCLUDE: - Develop rapport with individuals supported and foster their personal growth - Monitor and report umer\'s progress, challenges and status - Work individually with any program participants, which may include assistance/supervision with mobility, recreational activities or other needs.
