Job Description/Essential Functions:
Horizon House is seeking a Direct Support Professional (D.S.P.) with experience working with medically fragile individuals in a residential/community setting. D.S.P. provides support with daily living activities, community integration, medical appointments, religious events, and the development of Individual Service Plans (ISP) to individuals experiencing Mental Health/Intellectual Disabilities diagnosis. A D.S.P. will find assisting individuals with learning to live and function as independently as possible in the community rewarding.
Job Qualifications:
- High School diploma or GED and at least 2 years prior experience in the MH/IDD field or any combination of experience, education or training that would provide the level of knowledge, skill and ability required.
- Good communication skills, both oral and written.
- Ability to perform independently and as a team member.
- Ability to use a computer and document daily activities electronically.
- Required to possess a valid PA driver's license and maintain an acceptable driving record.
