Ministry Support (Diakon Lutheran Social Ministries) Case Manager II Positive Parenting Program in Norristown, Pennsylvania
Category/Shift/Hours per 2 week pay period: Full-Time / Varied Shift / 75 hours per pay period

General Responsibilities:
- To provide positive parenting program (PPP) services and case management to client population in accordance with state regulations and agency policies.
- Services are delivered in-home and in the community.
- Provides standard level of PPP, Teens PPP and Stepping Stones PPP.
- Completes PPP assessment tools and outcomes, session notes, monthly reports and encounter forms.
- Employs a range of social work interventions with client families to facilitate accomplishments of goals listed in the Service Plan.
- Makes initial case assessment and ongoing assessments.
Provides case management and face-to-face services as described in Service Plan, to meet stated client goals.
Education/Training: A BA or BS degree in social work, BSW, normally required. Trained and accredited in Positive Parenting Program (PPP) strongly preferred.
Experience: A minimum of two (2) years' experience in a related child welfare or human service position normally required. Experience in social work or case management is essential.
ADA: Ability to sit, stand, bend, stoop, reach, and lift items weighing up to 50 lbs.; visual and auditory skills.
