The Norfolk Community Services Board (NCSB) is the primary provider of public mental health, substance abuse, and intellectual disability services for the City of Norfolk. A continuum of services spans from outreach and prevention through intensive psychiatric care, provided by highly clinically oriented service professionals with expertise in these service areas.

Today, the NCSB provides treatment and rehabilitation services to over 6,000 Norfolk citizens yearly. The NCSB is seeking a Case Manager I who p rovides activities and services that assist adults diagnosed with a serious mental illness and co-occurring substance use disorders to access needed psychiatric, medical, substance abuse, social, educational, vocational, and other supports necessary to meeting basic needs. Will also complete clinical documentation in accordance with, agency guidelines.

Department Hiring Salary Range: $41,231 - $47,415

Essential Functions

Essential functions include but are not limited to:

  • Completes a face-to-face assessment to determine functional limitations, eligibility. for service, progress and need areas-at least once every 90-days. Face-to-face contact occurs through home visits as often as possible but may also occur-in-the office, or in another community setting: Initiates dialogue regarding psychiatric and physical health at all face-to-face visits as part of a holistic/integrated approach to care coordination.

  • Develops an Individualized Service Plan (ISP) based on assessment information in collaboration with the consumer and, if appropriate, family, or significant others. Reviews and updates the ISP at least every 90 days, or more often if needs change. Conducts a complete revision of the ISP at least annually.

  • Links to services, resources, and supports specified in the ISP. Examples include linking to- entitlements, substance abuse treatment, medical care, food, skill training, housing, etc.

  • Assists the individual directly with obtaining needed resources. Assistance also includes advocating for the-consumer to procure needed resources or services, while empowering the individual to be as independent as possible.

  • Coordinates services and service planning with other providers within NCSB and with other agencies, and private providers, such as psychiatric, hospitals, counselors, vocational programs, doctors, etc., and obtains clinical documentation from other providers to ensure continuity of care.

  • Makes contact with family or significant others to promote implementation of ISP and community stability.

  • Monitors progress, service delivery, physical and mental stability, and consumer satisfaction by providing monthly service to all individuals on assigned caseload. Provides increased frequency of service whenever clinically indicated.

  • Provides practical counseling and education to guide the individual in problem solving strategies and to develop a supportive therapeutic relationship. This may include encouraging compliance with treatment, substance abuse education, discussing relapse prevention strategies, stress management, etc. Performs crisis intervention to resolve critical situations.

  • Maintains chart documentation that follows agency guidelines and state regulations. Ensures that documentation substantiates billing by completing documentation and submitting filing according to program standards. Completes an accurate record of direct and consumer related service provided daily.

Education/Experience

Bachelor's degree in a human services field, or equivalent combination of training and experience.

Additional Information & Requirements

  • Valid Driver's License may be required depending on assignment.

  • Certification in basic first aid and CPR, depending on area of assignment.

  • Signing Bonus: This position is eligible for a one-time $5,000 signing bonus for new hires.

  • Retirement

    If you are hired October 5, 2010, or after to a position with City of Norfolk Retirement System benefits, you will be required to contribute five percent (5%) of your salary toward your retirement benefit. This will be a pre-tax payroll deduction.

    If you are hired January 1, 2022, or after to a position with Virginia Retirement System (VRS) benefits, you will be required to contribute five percent (5%) of your salary toward your retirement benefit. This will be a pre-tax payroll deduction.

  • The Tuition Assistance Program is established to encourage employees toward continued self-development and education. Permanent full-time and permanent part-time classified, unclassified permanent, full-time special projects, and constitutional employees who have completed six months continuous service will be eligible to apply. The applicant’s school of enrollment must be an accredited institution.
