Program: Apartment Program (CHOICES - Mental Health Housing Services)
General Description: Oversight of the cases of a team of Apartment Program Residents including budgeting, health management, and restorative service planning. Maintain and submit documentation for Medicaid billing for service.
Accountable To: Apartment Program Manager
Qualifications: Bachelor's Degree in Social Work, Psychology or related field. Position requires a clean, valid NYS Drivers License.
Salary Range: $38,355 - $42,764 annually
Hours: Full Time (40 hours)
