Job Overview:
The Lung Transplant NP is an advanced practice registered nurse who is trained to assess patients' needs, order and interpret diagnostic and laboratory tests, diagnose disease, formulate and prescribe medications and treatment plans,
The NP will work closely with physicians in both the inpatient and outpatient setting to manage, diagnose, formulate and prescribe treatment plans for transplant patients, and participate in transplant coordination,
Qualifications:
Valid NJ RN & NP license
Certified Clinical Transplant Nurse (CCTN) Certification preferred
Scheduling Requirements:
4 day work week
8am 6pm
Weekend coverage required
No holidays
Essential Functions:
Collaborate with multidisciplinary team/consultants to ensure patient receives optimum care,
Collection of health history and perform physical exam and documents all information clearly in medical record as necessary,
Participates in transplant coordination processes,
Documents timely and accurately in the medical record to reflect patient status and need
Ensures all necessary multidisciplinary notes are available in patient chart
Identifies the need for and provides staff education on a formal and informal basis
Order and interpret laboratory and diagnostic tests and follows up as necessary
Provides comprehensive disease and/or transplant education to ensure patient and family are able to care for patient
Responds to patient calls and questions regarding care or need for additional information/request
Sees patients in the office and/or response to patient questions by providing evidence based disease process management
Works with multiple departments and patient to coordinator the transplant evaluation process
Provides a compassionate and supportive environment with prompt and consistent service
Adheres to patient rights: provides for the confidential treatment of all communications and records.
Demonstrates professionalism with families, visitors, physicians, co-workers and supervisors.
Listens and communicates effectively.
Supports teamwork by cooperative problem solving through participation in meetings, projects, etc.
Utilizes principles of continuous quality improvement in all work situations to assess, measure and improve organizational and department functions
Organizes work sets priorities with a minimum of supervision and seeks guidance as appropriate
Makes decisions that are timely and consistent with department, objectives, policies and procedures.
Demonstrates sound judgment; accepts and benefits from constructive criticism.
Participates in ongoing education as required.
