Job Description

Pharmacist fill orders for medications, monitor patient medication therapies, provide drug information, identify, resolve, and prevent medication-related problems and supervise and direct support personnel in accordance with established policies, procedures, and regulatory standards. Pharmacists may interact with patients, family, and other healthcare providers as appropriate to ensure continuity of services and to achieve desired patient outcomes through safe, timely, cost-effective medication therapy.

Job Responsibility

  • Interprets medication orders and maintains accurate, complete patient medication profiles*

  • Prepares sterile products including large/small volume parenterals utilizing aseptic technique and monitoring parameters.

  • Proper storage and handling of hazardous medications, as per USP 800 guidelines

  • Selects, labels, and dispenses medications safely and accurately in accordance with established policies and procedures*

  • Provides clinical consultation and clarification to practitioners; recommends evidence-based medication therapy regimens and monitoring plans; and suggests appropriate, cost-effective therapeutic alternatives to medical staff as needed*

  • Implements and maintains clinical pharmacy services according to established policies, procedures, and protocols, consistent with government and agency regulations, and in collaboration with organizational goals and objectives*

  • Conducts and participates in quality improvement and research projects that align with departmental goals and objectives; collects and analyzes data and maintains documentation of assigned clinical activities

  • Facilitates the identification of medication use issues, implements, and communicates strategies promoting safe, appropriate, and cost-effective medication use, and develops methods for monitoring medication safety and quality*

  • Participates in the quality improvement and medication-use review activities of the department; collects data; conducts quality monitors and inspections; and maintains logs, records and other documentation as assigned*

  • Participates in committees as assigned by department leadership and collaborates with members of the inter-professional team to identify the needs of the clinical service

  • Participates in the development and implementation of treatment guidelines, protocols, formulary changes, and critical pathways as approved by appropriate committees*

  • Keeps the pharmacy leadership team abreast of all inter- and intra-departmental pharmacy related issues and activities

  • Documents all clinical activities and interventions accurately and completely in accordance with site-specific procedures*

  • Contributes to the advancement of the profession and of the department through attendance and/or presentations at local, state, and national meetings*

  • Provides accurate, adequate, and timely drug information and education to the interprofessional team, patients, and caregivers, as required*

  • Participates, coordinates, and facilitates educational, training and competency programs for members of the pharmacy team, including pharmacy students

  • Trains and monitors pharmacy team members in their delivery of patient care services*

  • Conducts educational programs for members of the interprofessional team*

  • Conducts pre-admission and discharge patient medications histories, as required

  • Monitors medication therapy regimens for contraindications, drug-drug interactions, drug-food interactions, drug-laboratory test interactions, allergies, appropriateness of drug and dose, and therapeutic duplications*

  • Reads, extracts, and interprets information in patient charts accurately*

  • Issues controlled substances to patient care areas and maintains records as required by law*

  • Receives drug orders and re-stocks Pharmacy

  • Re-stocks Omnicell as needed and assists with drug transfers as needed

  • Performs all Pharmacist related duties, as required.

  • Performs all duties of a licensed registered Pharmacist as required by the Director and as identified by the ability of the department to effectively provide comprehensive service. Maintains compliance with established procedures, policies, and laws in performing the duties of a licensed, registered staff pharmacist

Job Qualification

  • Bachelor of Science in Pharmacy, Pharm.D. or equivalent foreign graduate degree, required.

  • Minimum of one (1) year of hospital work, experience, required. ASHP-accredited post-graduate residency training, preferred.

  • Current license and registration to practice as a pharmacist in New York State, required.

  • BLS, PALS, and ACLS certification, required.

The salary range and/or hourly rate listed is a good faith determination of potential base compensation that may be offered to a successful applicant for this position at the time of this job advertisement and may be modified in the future.When determining a team member's base salary and/or rate, several factors may be considered as applicable (e.g., location, specialty, service line, years of relevant experience, education, credentials, negotiated contracts, budget and internal equity).

The salary range for this position is $105,400-$183,000/year
