Services for the UnderServed, Inc (SUS) Incident Management Specialist in New York, New York
The Incident Management Specialist is responsible for Incident Management for the Developmentally Delayed(DD) division and will abide by incident management regulations for a variety of regulatory agencies in order to ensure compliance with federal, state and local laws.
Ensure all incidents related to consumer safety are reported in compliance to regulatory funding sources.
Triage reports of incident to ensure all protective measures are in place for consumer safety
Ensure preventive measures are put into place to prevent the reoccurrence of the incident.
Coordinate the investigations of incidents and follow-ups of all incidents
Review incidents to ensure all follow-up are done in a timely manner as per funding regulations
Ensure that investigations are assigned and thoroughly completed and time frames are adhered
Act as the Quality Assurance liaison in the Incident Review Committee meeting for DD
Complete minutes/follow-ups for Incident review Committee meetings and submit to programs and appropriate staff.
review data for trends and ensure preventive measures are in place to address incidents
Responsible for conducting random checks of incident books for programs in preparation for program audits by funding sources
Responsible for compiling incident statistics on a monthly basis to compile quality reports annually
Responsible for analyzing any trends pertaining to consumer incidents and requesting appropriate corrective action in response to it.
Responsible for requesting additional information from external sources in order to thoroughly review incidents
Responsible for the management of data in the state computerized system, IRMA and/or other incident management databases.
Responsible for gathering documentation and sending out investigative packages to the Justice Center as require by regulations
Responsible for training staff on Incident Reporting and Justice Center Regulations
Conduct focus reviews at facilities based on corrective actions as per investigation recommendations

Bachelor’s Degree required in related field, Master’s Degree preferred.
Must be on call for incident reporting from programs during non-office hours, evenings, weekends and holidays
Minimum two (2) years of experience in human services in program operations
Experience working with consumers in programs which provide services to consumers with developmental disabilities
