Shift: 8:00 am - 4:00 pm, 4-day work week with on call responsibilities!
Pay Rate: $30 - $31.00/hr
Responsibilities and Duties:
To act as a habilitation specialist for individuals in the IRA program.
To supervise the implementation of the individual program plan for each resident of the facility.
To ensure the implementation of valued outcomes for each individual.
Schedule, conduct and document all interdisciplinary team meetings (in coordination with the day program if appropriate).
Ensure staff complete all required daily documentation including IRA Resident Habilitation Checklist, Daily Checks, etc.
Ensure all discipline specific assessments and evaluations are completed within a timely manner.
Ensure monthly IRA Day Habilitation note is completed on time.
Monitor monthly progress toward goal-oriented objectives.
Participate in all behavioral planning meetings.
Ensure the development and maintenance of an individual record-keeping system in accordance with regulations.
Maintain individual program planning section of the record.
Oversee and ensure the program planning process for each program participant in accordance with regulations.
As needed, work evenings and/or weekend hours to monitor provision of services to IRA individuals and to train direct care staff.
Advocate for the rights of the intellectually disabled.
Demonstrates an understanding of the principles or moralization and reflects this in program implementation.
Attend all meetings as required.
Act as liaison to the Human Rights and Informed Consent Committees
Perform any other duties as required by the Program Director or Assistant Program Director.
Performs duties according to the highest ethical and moral standards and in adherence with the facility's corporate compliance plan and all applicable governing laws.
Job Requirements:
Qualifications:
Bachelor's degree in one of the following areas: Social Work, Human Services, Special Education, Recreational Therapy, Psychology, Nursing, Occupational Therapy, Physical Therapy, Speech, or Vocational Rehabilitation.
One year's experience with MR/DD required preferably in an IRA setting (QIDP).
