Universal Health Services IOP SOCIAL WORKER (MA/LMSW) - New Bern in NEW BERN, North Carolina

Responsibilities

Horizon Health, the nation’s largest operator of hospital based behavioral health services, in partnership with CarolinaEast Health System in New Bern, NC is seeking a Social Worker for its IOP Behavioral Health Program.

Responsibilities include assessments, unit management, treatment planning and discharge planning, as well as group, individual and family treatment. Participates in referral development processes and activities as requested.

Essential Core Functions:

  • Responds to intake calls and completes admission paperwork. Assessments will be completed by RN, LPC or LMSW

  • Facilitates groups, family and individual sessions and completing all necessary documentation

  • Schedules Unit assessments, meetings and appointments as appropriate

  • Utilizes assessment tools including, but not limited to Geriatric Depression Scale, Zung, SLUMS, alcohol screening tool(s) under authorization/supervision of RN, LPC or LMSW

  • Communicates with referral sources including, family members, legal system, psychiatrists, primary care physicians, and other treatment facilities.

  • Reviews newly admitted patient accounts for eligibility and authorization daily

  • Maintains schedule for filing court paperwork and hearings. Works closely with court personnel to maintain good relationships

  • Participates as an active member of the interdisciplinary team

  • Addresses issues in a timely manner including crisis and higher level of care referrals.

    Qualifications

  • A Master’s Degree from an accredited college with a major in Social Work, Psychology, Sociology or similar field

  • Must have a valid state license in Social Work as required by the state in which the Social Worker will be providing services

  • Must have a LCSW (Licensed Clinical Social Worker)

  • Minimum three years of experience in a similar position

  • Proficient in software applications

  • Ability to manage stress appropriately

  • Ability to make decisions under pressure

  • Ability to manager anger, fear, hostility and/or violence of others appropriately

  • Maintains a positive attitude/public image while in the facility and in all work-related activities outside the facility

  • Excellent written and verbal communication and presentation skills

  • Ability to read and comprehend moderate to complex instructions and correspondence

  • Ability to write detailed correspondence

  • Ability to effectively present information in one-on-one and small group situations to patients and their families, Program leadership, employees, clients, providers, vendors, etc.
