Position Summary/Description:

The UNC Formerly Incarcerated Transition ( FIT ) Program is situated within the Department of Family Medicine at UNC Chapel Hill. The FIT Program is a partnership between UNC Family Medicine, the North Carolina Department of Adult Correction ( DAC ) (which administers the state prison system), Federally Qualified Health Centers, County Departments of Health, community-based organizations, and local reentry councils. The FIT Program coordinates with existing state and local resources to assist people coming out of incarceration with connections to essential health services. A specially trained Community Health Worker ( CHW ), with prior justice system involvement, acts as a peer navigator to connect to services. FIT was designed to leverage existing community and statewide investments in reentry and address the absence of linkages to essential healthcare for people coming out of incarceration that suffer from chronic disease, mental illness, and/or substance use disorder. FIT is modeled on an evidence-based program developed by the Transitions Clinic Network ( TCN ), which has demonstrated reduced utilization of emergency services, hospitalizations, and days incarcerated. FIT CHWs are integrated into their local community health center and assist clients upon reentry in establishing care at a primary care medical home, and if needed, obtain mental health and substance use disorder treatment. CHWs also assist clients with all aspects of reentry including housing, job training, legal needs, education, transportation, life skills, and interactions with the justice system. The mission of the FIT Program is to empower community members upon their return from incarceration to become self-sufficient and to reach their health and wellness goals. Since its inception, FIT has taken on additional projects that align with the program’s mission to improve health outcomes and decrease recidivism for formerly incarcerated people.

In 2022, the FIT Program launched a pilot, named FIT Wellness, to provide psychiatric and primary care services to individuals with serious mental illness ( SMI ) releasing from prison in Wake County. Given the success of the pilot, the state has awarded the program with funding to continue the work in Wake County and expand services to Orange, Durham, and New Hanover counties. The FIT Wellness program seeks to hire a full-time Program Manager. The Program Manager will have significant oversight of the implementation, development, budget, and management of the FIT Wellness program at all county sites. The Program Manager is responsible for expanding the FIT Wellness program to Orange, Durham, and New Hanover counties in addition to supporting the existing program in Wake County. Additionally, this position will supervise four full-time Community Health Workers within the Wake County FIT Program, based at the UNC REACH Enhanced Primary Care clinic in Raleigh, North Carolina. The position will function as the lead member of a highly integrated and interdisciplinary team and work closely with respective internal and external partner agencies.

Education and Experience:

  • Experience with data management, budgeting, and continuous improvement preferred.

  • Knowledge and experience in health issues related to criminal legal system involvement highly preferred.

  • Experience working with individuals with serious mental illness

  • Experience with client assessment, treatment planning, conducting interviews/investigations, advising/counseling clients, and participating in health promotion and health education activities.

  • History of involvement with the criminal legal system

Essential Skills:

  • Must have experience with project management, project startups, project implementation, or business development.

  • Must have experience communicating with diverse audiences and building relationships with a variety of partners and constituents.

  • Candidate should demonstrate the capacity to manage multiple responsibilities of a confidential nature and exercise a high level of discretion and independent judgment.

  • Candidate must have proven managerial, fiscal, and administrative abilities and an understanding of the culture, policies, and procedures relevant to academic and community health programs.

  • Candidates must exhibit vision and leadership qualities for expanding program.

  • Candidates must have experience supervising a team and familiarity of principles of human resource management

  • Must demonstrate strong time management skills and be able to prioritize timeliness in responding to internal and external partners
