Case Manager (Social Worker) - Telephonic and Home Visits

Who we need:

The Case Manager provides in-home, in-person care coordination in defined areas of Arizona and telephonic care coordination across the United States. Responsible for assessing, planning, implementing, monitoring, and evaluating options and services to effect an appropriate, individualized plan for the beneficiary across the continuum of care. Using independent judgment, the care manager utilizes clinical knowledge and competence, communication skills, problem solving and conflict resolution to effectively ensure optimal client outcomes with consideration to payer requirements. Also coordinates beneficiary needs related to community resources, financial assistance programs, and long-term activity of daily living needs outside the benefit criteria of the medical insurance coverage. Will work effectively with all health care team members internally and externally.

What you’ll do:

  • Use of independent clinical judgement and discretion within scope of practice to assess, plan, implement, monitor and evaluate options and services to effect an appropriate, individualized plan with the beneficiary across the continuum of care; utilizes clinical knowledge and competence, communication skills, problem solving and conflict resolution to effectively ensure optimal beneficiary outcomes with consideration to client requirements.

  • Develops beneficiary centered care plans demonstrating shared accountability between beneficiaries, caregivers, and providers by working collaboratively with the beneficiary, their provider, and other members of the Funds Health Care team as necessary to coordinate care, address identified gaps in care or compliance, with the goal to optimize beneficiary function, or prevent further decline in health, consistent with the Funds and KEPRO “caring” philosophy.

  • Maintains strict standards for client confidentiality and client related information. Complies with all organizational, state and federal regulations and policies on confidentiality.

  • Manages case load to ensure all required documentation and entry of assessment results into web-based database are completed accurately and timely in accordance with regulatory requirements and company policies and procedures.

  • Maintains open communication with all appropriate parties.

  • Pursues ongoing education, certification, and self-development to remain current with case management standards.

What you’ll need:

Required Qualifications

  • Bachelor’s degree in Social Work; and Active, unrestricted SW license in Arizona.

  • Preferred:

  • Master of social work (MSW)

  • Case Management Certificate (CCM)

Knowledge, Skills, Abilities

  • Ability to multi-task and prioritize with variable and sometimes conflicting deadlines; superior attention to detail and demonstrated ability in decision-making.

  • Demonstrated initiative and judgment in performance of job responsibilities, while maintaining professionalism, flexibility and dependability under pressure.

  • Strong communication (written/verbal), interpersonal, organizational, time management and communication skills with a strong focus on customer service, including building and maintaining relationships with internal/external customers and facilitating meetings.

  • Ability to work independently and as part of a team.

  • Ability to research/identify and apply appropriate standards of care.

  • Must be proficient in Internet/Web Navigation and research

  • Interest in continuous learning and a commitment to staying informed on regulatory changes.

  • Designated private workspace at home residence.

  • Ability to make home visits within contracted counties in Arizona.

Experience

  • Minimum 3 years Social Work experience w/older adult population in a healthcare environment
