Job Summary and Qualifications

Responsible to advance HCA clinical excellence and quality initiatives through clinical support of assigned Home Health or Hospice agencies (spanning multiple agencies, or provider numbers). Provides oversight of agency achievement of readiness and compliance with Medicare conditions of participation, state regulations, and accrediting body (e.g. The Joint Commission) standards.

  • Analyzes, evaluates, and reports service line clinical performance (as measured by service line clinical key performance indicators) to identify and prioritize needs or gaps in quality of care, patient experience and care efficiency of assigned agencies.

  • Based on performance trends, needs assessment, regulation updates, and organizational priorities, plans, coordinates, provides, training and on-site support for clinicians, Clinical Managers, Agency Directors.

  • Conducts on-site visits for continuous quality improvement to ensure clinical performance aligns with HCA policies, procedures, and standards. Works with subject matter experts (e.g. Operations, Education, Centers for Clinical Advancement, Survey Readiness, Regulations, Compliance, Appeals team) to establish succinct, prioritized performance improvement plans and agency plans of correction.

  • Delivers and/or coordinates state of the art, evidence based training, mentoring, guided learning, and education programs to support clinical and operational goals.

  • Serves as a clinical resource and clinical operations support to assigned agencies as needed, including providing orientation, onboarding, preceptor experience, and training for new hire and tenured colleagues; facilitating and conducting agency operations, and / or other duties as assigned to ensure patient care delivery.

  • Facilitates quality improvement and clinical initiative efforts as directed by the service line leadership.

  • Maintains current knowledge of industry / service line regulatory changes and disseminates this information to the service line leadership team.

  • Assures the highest standard of ethical, professional, and clinical judgement.

What qualifications you will need:

  • Bachelor or Masters Degree in Occupational Therapy, Physical Therapy, Nursing or a Masters Degree in Speech/Language Pathology from an accredited program, Health Care Administration, or related field required; Master’s Degree preferred.

  • Minimum five years’ experience within a licensed and certified (Medicare) home health or hospice agency in a leadership position HCA Healthcare at Home required; or equivalent combination of education and experience.

  • Excellent communication and teaching skills required.

  • Homecare Homebase: Point Care and R2 (back office), MS Office software and virtual meeting platform experience required.

  • Valid RN, PT, OT, or ST license within practice state(s) required.

  • Certified Hospice and Palliative Care Nurse preferred for Hospice Service Line and Certificate for OASIS Specialist – Clinical credential preferred for Home Health Service Line.
