Position Title: Neuro Services Nurse Navigator/Stroke Coordinator
Position Summary:
The Neuro Nurse Navigator will coordinate the stroke program and work within the multitude of other neuroscience services functioning as an advocate, educator and facilitator of care. Responsible for ensuring all referred patients receive quality and comprehensive services throughout the continuum.
What the Neuro Services Nurse Navigator/Stroke Coordinator will need:
Bachelor's degree in Nursing required. Master's degree preferred
Current State of Michigan Registered Nursing License, Certified Nurse in subspecialty (OCN,CNRN)
CPR or BCLS certified
A minimum of three (3) years' experience in acute care setting
What the Neuro Services Nurse Navigator/Stroke Coordinator will do:
Coordinate the stroke program to include a multi-disciplinary team of Physicians, Nurses and Ancillary staff
Perform ongoing stroke data collection to analyze, interpret and report for program process improvement and evaluation of clinical outcomes
Develop and implement stroke education for colleagues, patients and the community
Collaborate regionally with St. Mary's stroke team to enhance best practices opportunities
Work in compliance of Joint Commission stroke standards of care with the goal of achieving "Primary Stroke Center of Excellence."
Coordinate care for all patients requiring hospitalization and subsequent follow-up to avoid unnecessary readmissions this includes a "Stroke Support Group" with having patient and family guidance, education and support
Participates in the development and implementation of policies, procedures and protocols
