Westchester Community Health Center Patient Navigator in Mount Vernon, New York
Job Summary:
Under the general supervision of the Director of Medical Case Management, the patient navigator will perform all duties required below and report to the program supervisor.
Primary Responsibility of the Position:
Case findings Reassessment Self-management assessment Client assistance Client engagement Outreach for client re-engagement Linkage to services Accompaniment Health Education Complies and maintains necessary reports, records, and statistics as required by Public Health Solutions and NYC DOHMH Performs other related duties as assigned. Education and Experience:
Bachelors Degree At least 2 years of Case management experience Must have a valid NYS driver's license. Excellent writing skills Proficiency in Microsoft Excel, Word, and PowerPoint is highly desired.
