WellSpan Health Day Hospital Coordinator - York Child/Adolescent Day Hospital in Mount Gretna, Pennsylvania

Full-time, Days Job Description:

Under the direction of the program director and in concert with the other treatment team members, this staff coordinates the day-to-day functions that support program operations. This staff also manages the milieu, conducts individual, family, and group therapeutic interventions, performs primary case contact and management with family members and community agencies, and performs documentation related to caring for children and adolescents with mental health issues.

Duties and Responsibilities
  • Coordinates referral process, discharges, and assists in communicating transfers to other levels of care.

  • Coordinates staff schedules/caseloads, transportation, meals, and assists with petty cash fund.

  • Performs continued stay reviews with HMO’s.

  • Sends and tracks satisfaction questionnaires.

  • Assists with the completion and issue of the weekly status report.

  • Coordinates clinical interventions and space for family and group sessions.

  • Organizes or delegates special program events (i.e.: home school transitions, family nights).

  • Assists in completing performance improvement plans (i.e.: chart reviews).

  • Maintains current attendance records.

  • Coordinates and monitors volunteers and interns.

  • Completes client service activity, as needed.

  • Transports clients safely, as needed.

  • Assists treatment team members in planning and leading therapeutic groups and other activities.

  • Completes annual safety education requirements for the department.

  • Responds promptly and skillfully to limit potentially unsafe client behaviors.

  • Completes assessments (e.g., PIE, Conners, CBCL, CDI, releases) as needed.

  • Assists with medication administration and recording as directed.

  • Communicates relevant client progress and participates in weekly treatment team planning.

  • Addresses and resolves client funding matters.

  • Conducts inter-agency meetings as necessary.

  • Orients family members to nature of therapeutic services and the specific program.

  • Increases other family members understanding of client needs and progress.

  • Promotes positive family relations, especially effective parental strategies.

  • Collaborates in communicating appropriate treatment and discharge plans with the funding sources and the treatment team.

  • Completes progress notes, treatment plans, and discharge instructions/summaries, as needed.

  • Documents client’s functioning, response to treatment, behavioral changes, unsafe events (e.g., PERTS form), as well as family and agency contacts and decisions.

  • Minimum Experience:

  • 2 years

  • Minimum Education:

  • Bachelor’s Degree

  • Minimum Field Of Expertise:

  • Mental Health related field

  • Skills:

  • Requires substantial interpersonal verbal and non-verbal skills to build working alliance, empathize with, and respond to referral sources, patients, their families, and co-workers; Analytical ability necessary to manage several tasks simultaneously in a fast-paced mental health environment; Physical effort sometimes required in assisting, moving, or restraining agitated or immobile patients, along with the ability to walk or stand often; Visual ability required for normal patient interaction and to manage written information.

  • Must possess a valid driver’s license in the current state of residence. Driving record must meet requirements established by WellSpan Risk Management.
