MINIMUM QUALIFICATIONS: Ability to perform Essential Duties as outlined below. BA/BS in Human Service Field. One-year experience providing Service Coordination with VACT Program Ability to comply with all of company Policies and procedures. Ability to comply with Clients Rights. Ability to comply with company and Division departmental safety procedures. Ability to read, write and speak the English language. Ability to legally transport clients. Current CPR/First Aid Certificate or ability to be trained. _ PREFERRED QUALIFICATIONS: LSW or eligible for TLSW Two years experience providing Service Coordination with VACT Program __ ESSENTIAL DUTIES: Assist with screening clients referred to VACT Program Provide Intake services for all clients accepted into VACT Program and assign cases to Service Coordinators Assess clients needs and evaluate progress. Face to face contact with assigned clients at least 3 times a month in his/her home or community Keep treatment plans and reviews current for the assigned caseload. Initiates, coordinates and facilitates interdisciplinary treatment team meetings Provide individual and/or group supportive counseling and symptom management; motivates clients to work toward achievement of goals and objectives as outlined on the treatment plan. Use clear communication skills with clients and other professionals. Link clients with necessary resources and services. Adheres to ACT specific policies and procedures. Incorporates ACT language into all client related documents. Adhere to documentation standards. Actively participate in daily staff organizational meetings and treatment planning review meetings Provide crisis services and intervention and/or assistance with consideration of the principles of client choice and least restrictive alternatives. Adheres to company Standards of Conduct and Compliance. company, Medicaid, and other licensing agencies. Employee ensures that documentation (progress notes, SALs, timesheets, summaries, reauthorizations, etc) are completed accurately and timely and will reflect services the client(s) has received as appropriate. Serve on committees as assigned. Attend all mandated trainings related to profession and enhance knowledge and/or skills which include Essential Learning Trainings. OTHER DUTIES AS ASSIGNED.
