Maxim Healthcare Services Licensed Vocational Nurse (LVN) Private Duty Nursing in Morgan Hill, California

Maxim Healthcare is looking for a Licensed Vocational Nurse (LVN)/Licensed Practical Nurse(LPN) who assumes responsibility and accountability for the application of thenursing process and the delivery of patient/client care. The LVN/LPN is alsoresponsible for providing services, as directed by contract and/or program,while under the supervision of a contract/program Registered Nurse or clinicaldesignee. The LVN/LPN provides service in accordance with Maxim’s philosophy,standards and policies.
Essential Duties and Responsibilities:
Utilizes the nursing process to plan, implement, and evaluatepatient/client care and/or services provided to client(s) receiving contractedservices, as required
On each assignment, provides skilled nursing care/servicesin accordance with prescribed orders
Identifies and documents changes in patient/clientphysiological and psycho-social condition
Notifies Director/Supervisor, and contracted customerdesignee if applicable, when:
Physician notification is required in order to obtainnecessary orders for intervention(s), per state regulations
Changes in patient/client condition have occurred
Adheres to Maxim policy, procedure and applicable protocols
Identifies and prioritizes actions based upon patient/clientcare requirements and/or contracted customer requirements
Ensures that documentation reflects patient/clientcollaboration including participation in plan of care creation and maintenance,physician correspondence/collaboration, expected patient/client outcomes,nursing interventions employed, and other items as required by contractedcustomer
Performs/provides interventions, treatments, and medicationadministration, according to identified priorities, the plan of care, facilitypolicies and patient/client care outcome standards as applicable
Monitors and documents effectiveness of interventions,treatments, and medications
Assures patient/client safety at all times
Monitors the safety and effectiveness of the care/services environmentand equipment used, including universal precautions
Executes consistent, effective interpersonal communicationsand maintains positive, professional relationships with patient/client andcontracted customer stakeholders
Adheres to the following professional practice standards:
Nurse Practice Act
Standards of Nursing Practice
Standards of Patient Care
Standards of Accrediting Body
State and Local Laws
Patient Bill of Rights
Policies and procedures of contracted customers
Adheres to Maxim policy, procedure and applicable protocols
Attends case conferences, team meetings and educationin-services, as required
Performs other duties as assigned/necessary
Minimum Requirements:
Graduate of an accredited practical nurse or vocationalnursing program preferred
Currently licensed as a Licensed Practical Nurse or LicensedVocational Nurse license in specific state required
Complies with accepted professional standards and practicerequired
One year prior professional nursing experience preferred
Willingness to participate in skills advancement training ornovice nurse program if applicable
Current CPR, as applicable, for direct patient care
PPD or chest x-ray if applicable
Successful completion of Maxim Orientation and Compliance Training,and all other assigned trainings as applicable required
Successful completion of Maxim’s Competency Program required
Proficiency in the English language required
Current Health Certificate (per facility contract or stateregulation) required
Must meet all federal, state, local, and contractrequirements
