Responsibilities Provide rehabilitative services to individuals with SMI, focusing on skill development, psychiatric stability, social competencies, personal and emotional adjustment, independent living, parenting skills, and community integration Collaborate with clients to develop individualized treatment plans that address their unique needs and goals Conduct comprehensive assessments to identify areas of strength and areas for improvement Deliver evidence-based interventions and therapeutic techniques to support clients in achieving their rehabilitative objectives Monitor and evaluate clients' progress regularly, adjusting treatment plans as necessary Document all services provided, including progress notes, treatment plans, and any relevant client information, in accordance with clinic and state guidelines Work independently effectively managing time and caseloads Collaborate with a multidisciplinary team, including therapists, case managers, and psychiatrists, to ensure coordinated and comprehensive care for clients Assist clients in arranging crisis assessments, interventions, and stabilization services, when needed Provide individual and group interventions, ensuring weekly client encounter targets are met Participate in clinical and administrative supervision of cases through individual supervision, weekly team meetings, trainings, and field evaluations of care Meet annual requirements for ongoing mental health training as mandated May act as an interpreter for psychiatry, medical, or other mental health outpatient clinic service visits This position requires a background check, a negative TB test, and a Hepatitis B vaccination. Qualifications All required qualifications must be documented on application materials Required Qualifications BA/BS in psychology, social work, counseling or a related field, plus at least 2 years of experience or a combination of related education and work experience to equal at least 6 years. Experience working with individuals with serious mental illness or similar populations Possession of a valid Minnesota driver's license and access to a reliable vehicle with insurance coverage to facilitate home and agency visits, as well as transportation of clients as needed Fluency in written and spoken English to effectively communicate with clients and team members Proficient in documentation and record-keeping in an electronic health record system, ensuring compliance with clinic and state regulations Licensure Requirements: Lice sure is not required unless you hold a Social Work degree (BA, BA, or MA). A Social Work license (LGS, LGSW, LISW, or LICSW) is required only for candidates who hold a Social Work degree (BA., BS., or MA), per the Minnesota Board of Social Work requirements. Preferred Qualifications Master's degree in social work, psychology, counseling or related field Excellent communication and interpersonal skills, with the ability to establish rapport with diverse individuals Demonstrated ability to create and implement effective treatment plans based on client needs Experience providing community services (i.e. case management) Language proficiency in one of CUHCC's dominant languages (written and spoken) Spanish, Somali, or other cultural groups- Lao or Vietnamese Experience working in a community mental health clinic BIPOC and LGBTQ+ individuals are encouraged to apply The desired work schedule for this position is Monday through Friday during daytime hours, with some early evening availability. The work schedule will be a hybrid model, including a combination of time spent in the office, remote work, and community-based.
