About the Role: The Fatherhood Coordinator will be responsible for supporting the coordination and monitoring of services related to whole-person, integrated care. Through programming, which includes groups, events, outreach activities, and individual case management, the Fatherhood Navigator will provide coaching, resources, and services that prepare fathers to successfully navigate healthcare systems and the physical, emotional, and spiritual needs throughout the parenting journey. The Fatherhood Coordinator will primarily work with African American expecting and parenting fathers. The Fatherhood Coordinator will also participate in outreach activities to engage the community, conduct health screenings, and bridge care for families. Qualifications: Bachelor's degree in Public Health, Human Services, or a related field and two (2) years of related experience working in a community health and human services environment. Associates degree in Public Health, Human Services, or a relat d field and four (4) years of related experience working in a community health and human services environment. High School Diploma or GED with six (6) years of related experience working in a community health and human services environment. Two (2) years of experience coordinating culturally based program and service responses to individuals from marginalized communities. Three (3) years of experience working on projects and/or programs related to healthcare, public health, and/or social determinants of health initiatives. MNsure Certified, preferred. If uncertified, must complete certification within three (3) months of hire. Must be able to complete training courses related to fatherhood as assigned by supervisor within twelve (12) months of hire, required. Demonstrated knowledge and ability to plan and implement community-based events. Excellent verbal and written communication skills with a high-level attention to detail, maintaining clear and effective dialog with all levels of employees, guests, and the community. Ability to form and maintain internal and external partnerships and networks that support the mission, vision, values, and goals of the organization. Advanced PC skills in Microsoft Office, including Word, Excel, PowerPoint, Teams as well as proficiency with various technology including mobile devices. Valid driver's license with good driving record, proof of insurance, reliable vehicle and the ability to meet NorthPoint's driving requirements.
