UMPhysicians Community Health Worker (Primary Clinic) in Minneapolis, Minnesota

The Community Health Worker provides community based support for individuals in a clinic based care coordination program and works within a team of primary care physicians, behavioral health clinicians, and social workers. Community Health Worker Overview Key responsibilities include, but are not limited to: Promote wellness by providing culturally appropriate health information to patients and providers while educating patients on disease prevention, self-management of chronic illnesses and medication adherence Bridge the gap between communities and the health and social service systems by enhancing care quality, aiding communication between health system(s) and patient to clarify cultural practices and health information Increase access to primary care through culturally competent outreach and enrollment strategies, making referrals and coordinating services to build/enhance patients' knowledge, skills, and confidence to access resources and obtain care Enroll patients into programs such as health insurance and public assistance Build Individual and Community Capacity Minimum Qualifications: Prior experience as a community health worker, peer specialist, care manager assistant or similar. Experience, competence and sensitivity in working with diverse communities. Fluent in English.
