Ascension Health Social Worker - Behavioral Health in Milwaukee, Wisconsin

We Are Hiring

Social Worker - Behavioral Health

  • Columbia St. Mary's Hospital - Milwaukee, WI

  • Part-time, 32 hours/week

What You Will Do

Investigate, assess, and plan interventions to help patients cope with social, emotional, economic and environmental problems.

  • Interview patients, assess priorities, and document case activity.

  • Consult with team managers, statutory and voluntary agencies and patient's relatives to ensure compliance with patient's treatment plan.

  • Assist clients/families with obtaining community assistance by referral to proper resources.

  • Develop and maintain working relationships with community health, welfare and social agencies and seeks creative means to assist the client with his/her needs.

  • Support management of concurrent review, discharge planning processes, social services, home care and other post discharge needs.

What You Will Need

Licensure / Certification / Registration:

  • One or more of the following required:

  • Social Worker credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.Licensure is dependent on state associate works in.

  • Social Worker credentialed from the Minnesota Board of Social Work obtained prior to hire date or job transfer date.Licensure is dependent on state associate works in.

Education:

  • Bachelor's degree in Social Work required.

Work Experience:

  • 3 years of experience preferred.
