PROGRESSIVE COMMUNITY HEALTH CENTER Behavioral Health Navigator in MILWAUKEE, Wisconsin

Progressive Community Health Centers is seeking an experienced professional to serve as a Behavioral Health Navigator at our Lisbon Avenue Health Centers. Does not require working late nights, no holidays, and no weekends! Who are we? We provide high-quality, comprehensive care to roughly 16,000 patients annually majority of who are low-income. No one is ever turned away from our clinics based on insurance status or ability to pay. We value a patient-centered philosophy and provide a range of services that include family medicine, internal medicine, women\'s health, urgent care, dental, radiology and integrated behavioral health. We also offer supportive services such as case management, health education, and benefits enrollment. What will you do? The Behavioral Health Navigator involves implementing care plans designed by social workers to address patients\' identified needs, resources, and goals. Key responsibilities include educating patients and their families about available options, prioritizing cases based on urgency, and effectively utilizing community resources. The position promotes patient self-management and independence, assisting with essential services like transportation and housing. Additionally, the role requires maintaining accurate documentation, participating in departmental meetings, and fostering collaborative teamwork while adhering to organizational policies and ethical standards. The candidate must possess strong communication skills, cultural competency, and the ability to work independently and as part of a team. Knowledge of federal and state regulations, particularly regarding patient confidentiality and HIPAA, is essential. Schedule: Monday - Friday, 8 a.m. - 5 p.m. Responsibilities Implements the plan developed by the Social Worker that addresses the patient\'s identified needs, issues, resources, and care goals; determining the choices available to individual patients; and educating the patient/family on available options; prioritizing patients according to intensity, need, and required follow-up. Identifies and effectively utilizes community resources to meet the needs of patients/families. Promotes patient self-management and empowers patients and families to achieve maximum wellness and independence.
