JOB REQUIREMENTS: Position Description: The Medical College of Wisconsin\'s Department of Medicine, Division of Gastroenterology is seeking a Nurse Practitioner or Physician Assistant to support a multidisciplinary health care team, providing a broad range of diagnostic and therapeutic services for the patients of the IBD Outpatient services located in Milwaukee, Wisconsin at Froedtert Hospital Specialty Clinics. In this role you will work in a shared practice model with one or more faculty members, as well as independently, within the sub-specialty of Inflammatory Bowel Disease (IBD) clinics. The APP will be responsible for new and follow-up visits, in-basket management, and evaluating other acute patient issues. The Advanced Practice Provider (APP) works as a member of a multidisciplinary health care team who will contribute to excellence in program administration, patient care, patient safety, research, quality improvement, and teaching. Areas of responsibility will include, but not be limited to providing direct patient care to adults patients in a clinic setting as a member of the Gastroenterology IBD specialty services. Core Advanced Practice Responsibilities Perform comprehensive histories and physical exams, identifies normal and abnormal findings, including physical, cognitive and psychosocial status. Order, perform, and interpret screening and diagnostic procedures and tests, and appropriately communicates and documents information Write accurate and timely progress notes, procedure notes, and/or admission and transfer orders Develop pharmaceutical and non-pharmaceutical treatment plans. Monitor effectiveness of plan of care Educate patients on disease and treatment plans. Perform procedures as trained and appropriately credentialed. Document within the Electronic Medical Record (EMR) and completes other necessary documents. Collaborate and/or consults with physicians and other health care providers as appropriate. Educates, counsels, and assists patients and families regarding current health problems, health maintenance activities and preventive behaviors. Attend required training and meets key regulatory requirements, including those of the privileging institution, CMS, and the Joint Commission.
