Edward M. Kennedy Community Health Center is one of the largest community health centers in Massachusetts serving Worcester, Framingham, Milford, and the surrounding communities.  We are a thriving and growing organization, and our team is expanding across sites to support this growth.

We are currently hiring an OBAT Data Coordinator based in Milford. This position will be  responsible for the entry and tracking of data for the program and all funding agencies within the required time frames.

Essential Functions:

  • Communicates with the medical team and handles records (faxing, mailing, obtaining).

  • Conducts intake assessments for new clients and completes documentation on time.

  • Maintains electronic medical records, ensuring data accuracy and compliance.

  • Coordinates transportation for medical appointments.

  • Provides education on OBAT and recovery strategies.

  • Completes and submits outcome measurement tools as required.

  • Attends and supports data system training and assists staff in its use.

  • Utilizes data to achieve program goals and improve practices.

  • Creates and analyzes reports to identify trends and recommend improvements.

  • Reports findings and gaps using data and observations.

  • Supports quality measure initiatives and outreach strategies.

  • Schedules appointments and assists with outreach efforts.

  • Runs reports and supports team meetings as needed.

  • Assists with grant applications and site visits.

Required Qualifications:

  • Experience working with data and using data to make improvements

  • Ability to summarize data and report on trends

  • Strong attention to detail

  • Excellent oral and written communication skills

  • Collaborative working style

  • Proficient in Microsoft Office products Excel and PowerPoint
