Acute Home Healthcare, Inc is seeking a skilled and dedicated Physical Therapist (PT) to join our home healthcare team, serving patients across Metro Detroit and surrounding communities. This role offers flexible hours and immediate placement. As a home health PT, you will work closely with patients to improve their mobility, strength, and independence in the comfort of their homes. You will develop and implement personalized rehabilitation plans to help patients recover from injury, surgery, or manage chronic conditions.

Key Responsibilities:

• Patient Assessment: Conduct initial and ongoing assessments of patients' physical conditions, functional abilities, and rehabilitation needs.
• Care Planning: Develop individualized physical therapy plans focused on enhancing mobility, reducing pain, restoring function, and preventing disability.
• Therapeutic Interventions: Provide direct physical therapy treatments, including exercises, manual therapy, gait training, and therapeutic modalities.
• Education and Training: Educate patients and caregivers on home exercise programs, safety practices, and lifestyle modifications to improve long-term outcomes.
• Progress Monitoring: Track and evaluate patients' progress, adjusting care plans as needed to meet rehabilitation goals.
• Collaboration: Work collaboratively with other healthcare providers, including physicians, nurses, and occupational therapists, to ensure comprehensive patient care.
• Documentation: Maintain accurate and timely patient records, including treatment plans, progress notes, and communication with the healthcare team.
• Patient Advocacy: Provide support and encouragement to patients, helping them regain independence and confidence in their physical abilities.

Qualifications:

• License: Active, unrestricted Physical Therapist (PT) license in the state of Michigan.
• Education: Graduate of an accredited Physical Therapy program (Doctorate of Physical Therapy preferred).
• Experience: Experience in home healthcare or outpatient settings preferred, but new graduates are welcome to apply.
• Certifications: CPR certification required.
• Skills: Strong clinical skills, excellent communication, and the ability to work independently in patients' homes.
• Transportation: Reliable transportation required to visit patients' homes. Must have a valid driver's license and auto insurance.
• Background Check: Successful completion of a background check and drug screening required.
