This job is Remote, but requires a local candidate that can be onsite for training and presenting in the clinics. FTE: 1.000000 Shift: Shift 1 (Monday-Friday) Job Summary: The Behavioral Health RN Care Manager functions as a core member of a Collaborative Care team. The care manager is responsible for coordinating and supporting mental health care for patients whose conditions are managed by primary care providers. The care manager role educates patients about depression and its treatment and monitors depressive symptoms and patient\'s response to medication and/or psychotherapy using a structured instrument (e.g., the PHQ-9). The care manager works closely with the primary care providers to coordinate care with psychotherapists and consulting psychiatrists. Other duties as assigned. EXPERIENCE DESCRIPTION: A minimum of 3 years of clinical nursing experience, required. Experience working with patients with co-occurring mental health and physical health problems, preferred. Experience working with patients diagnosed with depression and/or receiving treatment for depression, preferred. Experience working with older adults, preferred. Experience working in an ambulatory setting working with diverse patient population, preferred. EDUCATION DESCRIPTION: Associate Degree in Nursing (ADN), required. Bachelor of Science in Nursing (BSN), preferred.SPECIAL SKILLS DESCRIPTION: Effective written and verbal communication skills. Demonstrated ability to establish rapport quickly with a wide range of people. Demonstrate ability to collaborate effectively in a team setting Knowledge of community resources Highly organized with the ability to manage time and prioritize work flow. Ability to be self-directed and work autonomously. Demonstrate effective verbal and written communication skills. A high degree of positive energy and the ability to rapidly initiate positive relationships with others to develop trust.
