FTE: 1.000000 Shift: 4 Schedule: 8 hour shifts between the hours of 0700-2100; Every third weekend Job Summary: As a member of the IPS team, you will be an essential point of contact for our Froedtert Pharmacy patients as well as internal and external staff, providing them with exceptional customer service and support telephonically or through communication platforms, such as MyChart. You will be responsible for handling patient and staff inquiries, resolving issues, and ensuring that all interactions are handled with professionalism and efficiency. This role is crucial in maintaining Froedtert Pharmacy\'s reputation for excellence and fostering positive relationships with our patients and staff. Key Responsibilities: * Customer Service: Serve as the first point of contact for customers and staff calling the pharmacy with inquiries, medication questions, and other concerns. Provide friendly, courteous, and knowledgeable assistance to ensure customer satisfaction. * Inquiry Resolution: Address concerns from patients or healthcare providers, aiming to resolve issues to their satisfaction. Coordinate with pharmacy staff to ensure efficient and effective problem resolution. * IPS Pharmacy Support Program (MTM): Conduct comprehensive medication reviews and assessments to ensure effective and safe use of medications. Assist patients and staff with insurance-related inquiries. Collaborate with healthcare providers and patients to develop and implement individualized medication therapy plans. * Documentation and Recordkeeping: Maintain accurate and up-to-date documentation of patient interactions and interventions. * Cross-Functional Collaboration: Collaborate with pharmacy technicians, pharmacists, and other members of the healthcare team to facilitate seamless communication and coordination of care for patients. EDUCATION DESCRIPTION: Bachelor\'s degree in Pharmacy or Pharm.D. from an ACPE certified school of pharmacy is required. Successful completion of an ASHP accredited PGY1 or specialty residency is preferred. SPECIAL SKILLS DESCRIPTION: Demonstrates effective use of computers and applications (e.g. MS Word, Excel, Outlook) as appropriate to practice and communicate effectively. Active membership in the Pharmacy Society of Wisconsin (PSW) preferred. Active membership in the American Society of Health System Pharmacists (ASHP) and/or other national pharmacy organizations as approved by the Pharmacy Administration preferred. LICENSURE DESCRIPTION: Active Wisconsin Pharmacist License is required or eligible for and must obtain within 6 months.
