Baptist Memorial Assistant-Case Management in Memphis, Tennessee
Summary
Provides assistance to the case management and social work staff on an ongoing basis. Communicates, coordinates, disseminates information with team members for care coordination. Performs other duties as assigned.
Responsibilities
Support social worker/case manager with administrative and case management tasks, patient communication, and data entry.
Assist with the coordination of community resources and services for patients, helping to ensure that their needs are met in a timely manner.
Work collaboratively with patients, families, physicians, and nurses to ensure continuity of care.
Maintain and update client records, ensuring all documentation is accurate and up to date.
Other duties as assigned.
Completes assigned goals.
Requirements, Preferences and Experience
Education
Preferred: Prefer candidates with a degree in social work, psychology, or a related field. Experience in a similar role or a background in healthcare, social services, or counseling can also be beneficial. Knowledge of medical terminology and experience with health care coordination is a plus.
Minimum: High School Diploma Skill in communicating clearly and effectively using standard English language (verbal and written) to achieve high productivity, efficiency, and accuracy to complete job requirements.
Experience
Preferred: Prefer 1 (one)-2 (two) years of experience in healthcare or human services settings. Experience with direct client service, case management systems, and data entry is also beneficial.
Special Skills
Preferred: Possess strong organizational, communication, time management, and interpersonal skills. Efficiencies in Microsoft Office Software. Ability to work well with teams.
