Health First HFMG Practice VBC Specialist - Seaside Family Practice in Melbourne, Florida
POSITION SUMMARY
To be fullyengaged in providing Quality/No Harm, Customer Service and Stewardship by ensuring that patients receive recommended preventative screenings and medical assessments. The position contributes to risk adjustment activities that support compliance with regulatory and contractual requirements specific to Medicare.
PRIMARY ACCOUNTABILITIES
Analyzes metric reports abstracted from the electronic medical record and other data bases to identify opportunities in reaching patient-care goals and metrics.
Monitors clinical workflow efficiency and presents opportunities for improving and closing care gaps.
Conducts daily VBC huddles with practice staff and providers. Attends Pod Meetings and VBC Dyad Planning Sessions.
Consistently communicates performance in meeting periodic targets for patient care goals and metrics to all stakeholders.
Pro-actively schedules appointments, screenings, procedures, and reminder calls, ensuring any pre-appointment instructions are provided.
Provides follow-up and education with respect and regard foreach patient’sindividualized needs to ensure their participation in their care.
Accurately files documents in patient electronic medical record, ensuring any care gaps are closed.
- Assists with other duties specific to business unit as needed to support patient and valuebased care outcomes and promote the patient experience.
MINIMUM QUALIFICATIONS
· Education:High School diploma or GED
· Licensure:none required
· Certification:none required
· Work Experience:Minimum two (2) years of experience in a physician clinic or practice environment
· Knowledge/Skills/Abilities: · Intermediate knowledge of front and back physician office operations including medical terminology, patient procedures and insurance processes. · Proficiency with Microsoft Office including Excel and electronic medical database applications. · High attention to detail. · Must have ability to work in stressful environment with patients/families at various ages and levels of understanding. · Flexibility to manage multiple priority activities. · Strong critical thinking ability to analyze various situations and identify solutions
PREFERRED QUALIFICATIONS
• Education:Associate’s Degree
• Licensure/Certification:Current, active State of Florida LPN license, OR Medical Assistant Certification
PHYSICAL REQUIREMENTS - Sedentary
• Majority of time involves sitting or standing; occasional walking, bending, stooping
• Long periods of computer time or at workstation
• Light work that may include lifting or moving objects up to 20 pounds with or without assistance.
• May be exposed to inside environments with varied temperatures, air quality, lighting and/or low to moderate noise
• Communicating with others to exchange information.
• Visual acuity and hand-eye coordination to perform tasks
• Workspace may vary from open to confined; on site or remote
• May require travel to various facilities within and beyond county perimeter; may require use of personal vehicle
