WorkSource Oregon Supportive Housing Navigator in MEDFORD, Oregon
Bilingual candidates are eligible for an additional \$1.50 Bilingual Differential. This bilingual differential is not included in the maximum salary above. Position Summary This position works with clients to develop a housing plan that will lead to housing stability and create conditions of self-management leading to medical adherence. The Supportive Housing Navigator will participate on the team of providers, and may be the lead of that team. The role of the Supportive Housing Navigator includes assisting clients in finding housing placement, enhancing and developing client life skills, creating and reviewing client budgets. The Supportive Housing Navigator will link the client to specific programs as needed such as health insurance, social security benefits, and other medical, and social assistance programs. Ongoing advocacy and support are provided to the client in order to maintain access to medical assistance programs, and minimize any break in access to medical care. The Supportive Housing Navigator facilitates this access through the provision of referrals and additional coordination or advocacy as needed. Essential Functions & Responsibilities Advocacy Helping and empowering clients to develop and utilize independent living skills and strategies Facilitating individual or group vocational and life skills trainings for clients who are receiving housing assistance from HIV Alliance Conducting outreach to recruit owners, landlords, and property managers Presents and distributes information about the program at local service organizations, homeless shelters, and other housing providers Proactively seek out and develop relationships to encourage positive relations between the agency and other organizations. Compiling statistical reports. Psychosocial assessment of needs, identification of client strengths and development of individualized housing care plan that addresses barriers to obtaining and maintaining housing Assist clients in identifying and accessing community programs and other resources Provide direct one-on-one and phone housing care coordination services Conduct weekly home visits to assess housing stabilization Maintain client records in a timely manner Make appropriate referrals, including follow up and documentation Conduct phone and face-to-face assessments and annual reassessments Advocate with other case managers and other service providers on behalf of our clients Provide HIV education, including explanation of available services, and partner programs to all clients Monitoring of service delivery and reassessment of needs Provide client transportation for client appointments and other treatment needs as needed Develop and implement care plans that acknowledge trauma, both current and past Performs related duties and fulfills responsibilities as required Attends community meetings as assigned Meet with clients in person on a drop-in and scheduled basis For a complete job description, please visit: https://hivalliance.criterionhcm.com/jobs/51/1/#627
