The Liver Transplant Social Worker provides psychosocial and supportive intervention, consultation and education to patients/families to assure comprehensive services throughout a patient's hospitalization, leading to a successful transition for discharge or transfer. Utilizes the Social Work process to determine the individual patient needs and the appropriate community resources to assure continuity of care from hospital to home or another health care facility. Contributes to the training of Social Workers and other health care professionals to enhance the educational programs of the Loyola University Health System. Assesses the patient's mental and social functioning by providing a psychosocial screen/history, assessment, resource information, counseling and a discharge plan.

Responds to immediate requests and ensures a comprehensive medical plan

by communicating the psychosocial and/or discharge information to the medical

team verbally and through documentation in the patient's medical record.

Completes discharge plan requirements, i.e., appropriate transport forms,

authorization for release of medical information, contacting appropriate family or

agency representatives, etc.

Attends service line multi-disciplinary rounds, unit meetings and meetings

specific to the assignment responsibility to facilitate continuity of patient care.

Provides educational and psychosocial programs to patients and families and

enhances the educational programs of LUHS.
