KEPRO RN Clinical Assessor - (Hybrid Role) -Martinsburg West Virginia in Martinsburg, West Virginia

Position Overview

Acentra Health is seeking an RN to work within the West Virginia Aged and Disabled Waiver (ADW) Pre-Admission Screen (PAS) programs. The ADW RN performs education and initial/re-evaluation assessments of Aged and Disabled Waiver participants for the purpose of determining Program Medical Eligibility.

** Please note that this job will require driving and this indvidual will perform field assessments covering Berkeley, Jefferson, Morgan, and Hampshire. Best city locations include Martinsburg, Shepherdstown and Berkeley Springs.

Job Responsibilities

  • Conduct thorough ADW assessments by traveling to designated locations, meticulously documenting findings within specified timeframes. Your primary focus will be on field assessments in the designated coverage counties.

  • Play a pivotal role in scheduling ADW assessments, ensuring they are coordinated and executed within established timelines.

  • Actively participate in inter-rater reliability initiatives to contribute to the consistency and accuracy of assessment procedures across the team.

  • Provide valuable insights during denial hearings when required, leveraging your expertise to support the proceedings.

  • Facilitate effective communication by contacting physicians and facilities to verify and validate medical information, ensuring accurate assessment outcomes.

  • Additional Assigned Duties: Undertake various other duties as assigned to support the overall operational efficiency and effectiveness of the team, as needed.

Required Qualifications, Skills, Knolwedge and Experience

  • Minimum of 2 years of experience working with the aging population or individuals with disabilities, preferably within West Virginia's Aged and Disabled Waiver program and Aged Disabled Waiver CareConnection©.

  • Familiarity with West Virginia Aged and Disabled Waiver Policies: The ideal candidate should demonstrate a strong understanding of the policies and procedures associated with the West Virginia Aged and Disabled Waiver. Willingness to travel to both homes and facilities is essential.

  • Accurate Medical Assessment: Candidates should possess the ability to precisely evaluate medical conditions, symptoms, and functional capabilities.

  • Educational Background: A Nursing degree is required, with a preference for a Bachelor's degree.

  • Valid West Virginia Registered Nurse License: All applicants must hold a valid Registered Nurse license for the state of West Virginia.

Compensation

The compensation for this role is $60,000 to $65,000 annual base salary. Based on our compensation program, an applicant’s position placement in the pay range will depend on various considerations, such as years of applicable experience and skill level.
