LCMC Health SOCIAL WORKER EVENING/WEEKEND in Marrero, Louisiana

$2,000 sign on Bonus with 1 year work committment

The Social Worker Evening/Weekend is responsible for assisting the patients and family members in resolving environmental, financial, psychological, and social problems which affect the patient’s recovery and adversely affect the patient’s length of stay. This will include collaboration with the physicians and other staff as well as developing and maintaining good working relationships with community health, welfare and social agencies.

Job Summary

The Social Worker Evening/Weekend is responsible for assisting the patients and family members in resolving environmental, financial, psychological, and social problems which affect the patient’s recovery and adversely affect the patient’s length of stay. This will include collaboration with the physicians and other staff as well as developing and maintaining good working relationships with community health, welfare and social agencies.

Part I - Organizational Duties and Responsibilities (40%)

    Part II - Job-specific Duties and Responsibilities (60%)

  • Provides social work services to patients & their families for emotional/social problems.

  • Assess and interviews high-risk patients and families for emotional/ social/environmental problems precipitated by illness, hospitalization, disability, impending death or institutional need, that may interfere with the patients recovery and/or discharge from the hospital. (5%)

  • Provides needed counseling and makes referrals, to include but not limited, to post acute care providers, counseling, protective services, financial assistance, etc in accordance with acceptable social work practice. (5%)

  • Makes appropriate referrals to protective services for elderly, adults and children as needed. (4%)

  • Works in collaboration with patient/family & health care team to formulate plans for patient.

  • Coordinates transfers to continuity of care agencies such as nursing homes, home health, LTAC, Hospice, dialysis units, other hospitals, in accordance to departmental guidelines. (5%)

  • Coordinates interdisciplinary team rounds on assigned units and/or with assigned physicians and participates in biweekly departmental interdisciplinary rounds in order to formulate plans of the patient’s needs after discharge. Then disseminates information from these to other care providers to ensure continuity of care. (5%)

  • Proactively anticipates discharge needs to include DME, HH, medication,transportation arrangements for home, etc. and has discussions regarding discharge, and expected departure prior to the day of discharge. (5%)

  • Responds to direct physician referrals within 24 hours of referral. (4%)

  • Delivers and explains the follow up "Important Message from Medicare about your Discharge Rights" in the appropriate timeframe as mandated by CMS. (4%)

  • Gives patient/family choices of agencies for providers (SNF, home health, DME, and hospice, etc) and documents patient preference on chart as well as documentation that choice was given to pt. (4%)

  • Coordinates/participates in fly conferences as deemed appropriate. (4%)

  • Proactively anticipates discharge needs and works closely with the Care Coordinators and discharge planners to research coverage issues of anticipated needs. (4%)

  • Interacts with physician to discuss plan of care, documentation clarification, and obtain necessary orders to ensure continuum of care and anticipated discharge needs are met. (4%)

  • Conducts an initial screening review of the pt and performs follow up visits as needed to interact with patients/families on the plan of care, goals, d/c needs, transfers to alternate levels of care and/or potential financial obligations. (4%)

  • Documentation

  • Opens cases and documents assessments within 3 days of admission on all Medicare eligible patients. (4%)

  • Opens cases and documents assessments within 2 days of admission on all SP patients. (4%)

  • Documents all interactions and interventions with patients, family and referral agencies concurrently in the medical record to include but not limited to, d/c arrangements, potential financial obligations, interpreter services utilized, fly conferences, interdisciplinary rounds, and any other issues pertinent to the continuum of care for the pt. (4%)

  • Works with Care Coordinators to provide the pt and/or fly with any pertinent letters or documentation regarding any financial obligations, adverse determinations and benefit coverage issues. (4%)

  • Maintains good working relationships with community health, social and welfare agencies.

  • Identifies and makes appropriate referrals to community agencies for patients and families social, financial and emotional needs. (4%)

  • Provides feedback to community agencies about resources needed and keeps up to date with the referral availability and methodology. (4%)

  • Participates in in-services provided by DME, Home Health and outside facilities and companies to learn about resources available in the community. (4%)

  • Intradepartmental participation

  • Completes CBT's timely , as well as all testing and licensure required for annual evaluation. (3%)

  • Attends unit staff meetings at least quarterly and provides update to supervisor on outcome of meeting. (3%)

  • Takes lead and back up call on weekends and holidays as per rotation schedule, and assists as needed to fill in vacancies. (3%)

  • Attends and participates in department staff meetings. As needed, participates in yearly retreat when scheduled. (3%)

  • Performs special assignments to include but not limited to, committee involvement, PI projects, training of new staff, etc. (3%)

    Job Specifications - Knowledge and Skills

    Required: 1. Basic computer skills 2. Excellent written, verbal and communication skills. Preferred: 1. Experience in a hospital setting 2. Medical Terminology

    Licensure(s): (LMSW) Licensed Masters Social Worker or above Related Experience (Specify months or years): Six months of directly related experience preferred.
