UMPhysicians Nurse Practitioner or Physician Assistant - Palliative Care in Maplewood, Minnesota

University of Minnesota Physicians is seeking a Certified Nurse Practitioner or a Physician Assistant to join our Palliative Care practice in Maplewood and Woodbury, MN. This position will work independently and in collaboration with other advance practice providers and physicians within the inpatient setting. In This Role, You Will: Join a team of 7 physicians and 16 advance practice providers that serve 6 hospitals and 9 clinics in the Twin Cities area. See patients in an inpatient setting 4 days every week primarily at M Health Fairview St. John's Hospital in Maplewood, MN and occasionally at M Health Fairview Woodwinds Hospital in Woodbury, MN, with the expectation that you may float within the metro M Health Fairview Hospitals, if needed. See approximately 9 patients/day. Provide inpatient visits, care, assessments, consultations and follow up for palliative care patients. Will see a variety of patients with chronic illnesses, with the exception of pediatric patients. Communicates with patients and families to explain diagnosis, procedures, expected diagnostic or therapeutic treatments and prognosis. Collaborates, consults and communicates with a multidisciplinary team of healthcare providers and staff throughout the continuum of care. Utilize a fully integrated electronic medical record (EPIC) is used. Schedule: .8 FTE Monday - Friday, 8am to 5pm. One scheduled day off a week. No nights, weekends or holidays. There is some flexibility with the schedule. No Call We're Looking For a PA or NP Who Is: A graduate of an accredited Physician Assistant or formal Adult/Gero, Acute Care or Family Nurse Practitioner training program, or a graduate of an accredited Physician Assistant Program. Eligible to be or is certified through NCCPA, ANCC or AANP. Licensed or eligible to acquire a license and prescriptive authority through the MN Board of Medical Practice (PAs) or licensed or eligible to acquire a license and prescriptive authority through MN Board of Nursing (NP's). Able to meet threshold criteria/qualifications for credentialing and privileges. Exceptional collaboration abilities, self-motivated and detail oriented. What Could Set You Apart? Palliative Specialty Certification (ACHPN or CAQ-PMH) or willingness to work towards certification. Previous experience in palliative care.
